Инсулин деглудек/инсулин аспарт ? первый комбинированный препарат базального и прандиального аналогов инсулина

Аннотация

Достижение оптимального гликемического контроля ? основная задача для предупреждения и замедления прогрессирования диабета и связанных с ним осложнений. Инсулинотерапия является самым эффективным способом достижения целей гликемического контроля. В свою очередь, оптимальная инсулинотерапия должна максимально точно имитировать физиологический профиль секреции инсулина, наблюдаемый у людей без сахарного диабета (СД). В связи с этим, разработка и внедрение аналогов инсулина, обладающих улучшенным профилем безопасности, предоставляет новые возможности в лечении диабета. Инсулин деглудек/инсулин аспарт (ИДегАсп, Райзодег?, Ново Нордиск, Дания) ? первый растворимый комбинированный препарат инсулина, содержащий 70% инсулина сверхдлительного действия деглудек и 30% аналога инсулина ультракороткого действия аспарт в одной инъекции, обеспечивающий потребность как в базальном, так и в прандиальном инсулине. В настоящем обзоре представлены результаты по эффективности, безопасности, переносимости и клинических преимуществах ИДегАсп. По данным программы клинических исследований, ИДегАсп обеспечивает достижение успешного контроля с преимуществом в отношении снижения содержания глюкозы плазмы натощак (ГПН) при использовании меньших доз и меньшего количества инъекций, а также ассоциируется со значительно меньшей частотой подтвержденных гипогликемий, в том числе ночных, по сравнению с существующими предварительно смешанными и базальными аналогами инсулина, а также обычной базис-болюсной терапией (при использовании вместе с инсулином аспарт). Результаты клинических исследований показывают, что ИДегАсп может быть логичным и обоснованным выбором для начала и интенсификации инсулинотерапии при СД 1 и 2 типа у пациентов, не достигших адекватного контроля при использовании максимальных доз пероральных сахароснижающих препаратов, а также простой альтернативой базис-болюсной терапии у пациентов, нуждающихся в интенсификации инсулинотерапии, особенно, когда соблюдение более сложного режима является трудновыполнимой задачей.
