Благодаря этому фокус терапии сместился от простого снижения гликемии к комплексной органопротекции, что обусловлено растущим пониманием взаимосвязи между гипергликемией и развитием сердечно-сосудистых, почечных и печеночных осложнений. Современные клинические рекомендации подчеркивают важность сочетанного применения сахароснижающих препаратов с дополнительными органопротективными свойствами, таких как ингибиторы натрий-глюкозного котранспортера типа 2 (иНГЛТ2) и агонисты рецепторов глюкагоноподобного пептида-1 (арГПП-1). Вместе с тем отмечается, что для пациентов с низким и умеренным риском осложнений и при ограниченных ресурсах сохраняется актуальность традиционных препаратов, в первую очередь метформина и производных сульфонилмочевины (ПСМ).</p><p>В настоящем обзоре проведен анализ современных рекомендаций по лечению СД2 и подробно рассмотрены преимущества и ограничения ПСМ, а также фиксированной комбинации глибенкламида и метформина («Глюкованс®»), которая обеспечивает синергический эффект, высокую сахароснижающую активность, экономическую доступность и удобство применения, что особенно важно для пациентов с выраженной гипергликемией и низкой приверженностью к терапии. В статье обсуждаются показания к назначению фиксированной комбинации в зависимости от фенотипа СД2, проводится сравнение плюсов и минусов «старых» и «новых» препаратов, оцениваются возможности сочетания ПСМ и метформина с современными органопротективными препаратами. Препарат «Глюкованс®» остается востребованным и эффективным инструментом в арсенале врача-эндокринолога, особенно в условиях необходимости баланса между эффективностью, безопасностью и стоимостью лечения.</p></abstract><trans-abstract xml:lang="en"><p>The evolution of type 2 diabetes mellitus (T2DM) treatment has contributed to the emergence of a disease-modifying, cardio-renal-hepatocentric approach in the global medical community. This has shifted the focus of therapy from simple glycemic control to comprehensive organ protection, driven by a growing understanding of the relationship between hyperglycemia and the development of cardiovascular, renal, and hepatic complications. Current clinical guidelines emphasize the importance of combining hypoglycemic agents with additional organ-protective properties, such as sodium-glucose cotransporter type 2 inhibitors (SGLT2 inhibitors) and glucagon-like peptide-1 receptor agonists (GLP-1 agonists). At the same time, it is noted that for patients with low and moderate risk of complications and limited resources, traditional medications, primarily metformin and sulfonylurea derivatives (SU), remain relevant. This review analyzes current recommendations for the treatment of type 2 diabetes mellitus (T2DM) and provides a detailed discussion of the advantages and limitations of sulfonylureas (SUs), as well as the fixed-dose combination of glibenclamide and metformin (Glucovance). This combination offers a synergistic effect, high glucose-lowering activity, cost-effectiveness, and ease of use, which is especially important for patients with marked hyperglycemia and low adherence to therapy. The article discusses the indications for prescribing the fixed-dose combination based on the T2DM phenotype, compares the pros and cons of “old” and “new” medications, and evaluates the potential for combining sulfonylureas and metformin with modern organoprotective agents.</p><p>Glucovance remains a popular and effective tool in the endocrinologist’s arsenal, especially when balancing treatment efficacy, safety, and cost.</p></trans-abstract><kwd-group xml:lang="ru"><kwd>сахарный диабет 2 типа</kwd><kwd>пероральные сахароснижающие препараты</kwd><kwd>препараты сульфонилмочевины</kwd></kwd-group><kwd-group xml:lang="en"><kwd>type 2 diabetes mellitus</kwd><kwd>oral hypoglycemic agents</kwd><kwd>sulfonylureas</kwd></kwd-group><funding-group><funding-statement xml:lang="ru">Работа выполнена по инициативе авторов без привлечения финансирования.</funding-statement></funding-group></article-meta></front><body><sec><title>Исторические подходы к лечению сахарного диабета 2 типа</title><p>За последние десятилетия подходы к лечению сахарного диабета 2 типа (СД2) претерпели значительные изменения, отражающие растущие знания о патогенезе заболевания и понимание влияния гипергликемии на состояние сердечно-сосудистой системы, почек и печени.</p><p>Традиционный подход к лечению СД сосредоточивался исключительно на контроле уровня глюкозы крови. Основной целью терапии было достижение целевых значений гликированного гемоглобина (HbA1c), поскольку считалось, что именно хроническая гипергликемия вызывает большинство осложнений СД [<xref ref-type="bibr" rid="cit1">1</xref>]. Однако клинические исследования показали, что контроль гликемии не гарантирует полного предотвращения развития сердечно-сосудистых заболеваний (ССЗ). Таким образом, глюкоцентрический подход оказался недостаточным для адекватного управления заболеванием, особенно учитывая высокую распространенность ССЗ среди пациентов с СД [<xref ref-type="bibr" rid="cit2">2</xref>].</p><p>Следующим этапом стало осознание важности профилактики кардиоваскулярных осложнений. Кардиоцентрический подход подчеркнул необходимость оценки риска развития ССЗ наряду с контролем уровня глюкозы. Пациентам предписывалось мониторировать и корректировать артериальное давление, уровень липидов и придерживаться рекомендаций по модификации образа жизни для снижения кардиоваскулярных рисков [<xref ref-type="bibr" rid="cit3">3</xref>]. Однако даже этот подход имел ограничения, поскольку не учитывал микрососудистые осложнения СД2, в том числе нефропатию [<xref ref-type="bibr" rid="cit4">4</xref>].</p><p>Кардиоренальный подход включал оценку функций почек вместе с управлением уровнем глюкозы и профилактикой ССЗ. Появились новые классы препаратов, такие как ингибиторы натрий-глюкозного котранспортера типа 2 (иНГЛТ2) и агонисты рецепторов глюкагоноподобного пептида-1 (арГПП-1), которые продемонстрировали эффективность не только в снижении уровня глюкозы, но и в кардио- и нефропротекции [<xref ref-type="bibr" rid="cit5">5</xref>]. Эти препараты были одобрены для использования в клинической практике благодаря результатам крупных клинических исследований, показавших снижение риска серьезных сердечно-сосудистых событий и прогрессирования хронической болезни почек (ХБП) [<xref ref-type="bibr" rid="cit6">6</xref>].</p><p>Современный кардио-рено-гепатоцентрический подход к лечению СД включает в себя управление рисками для всех трех органов-мишеней: сердца, почек и печени. Печень играет важную роль в метаболизме углеводов и жиров, а стеатоз печени, часто сочетающийся с СД2, увеличивает риск гепатоцеллюлярной карциномы и цирроза [<xref ref-type="bibr" rid="cit7">7</xref>]. При таком подходе используются стратегии комплексного контроля факторов риска, включая изменение образа жизни, фармакотерапию и мониторинг состояния здоровья пациента. Цель состоит в достижении оптимального баланса между снижением уровней глюкозы, артериального давления, холестерина и поддержанием нормального функционирования органов-мишеней [<xref ref-type="bibr" rid="cit8">8</xref>].</p><p>Таким образом, развитие методов лечения СД отражает глубокое понимание взаимосвязанности разных аспектов патологии и влияния каждого органа на общий исход заболевания.</p><p>Вместе с тем в погоне за органопротекцией не стоит забывать о пользе качественного снижения гликемии «здесь и сейчас»: по данным UKPDS, снижение HbA1c на 1% приводит к снижению риска инфаркта миокарда на 14%, инсульта — на 12%, смерти, связанной с СД, — на 21%, микрососудистых осложнений — на 24% [<xref ref-type="bibr" rid="cit9">9</xref>].</p></sec><sec><title>Обзор позиции современных клинических рекомендаций</title><p>Современные рекомендации подчеркивают важность сочетанного подхода, направленного на защиту сердца, почек и печени одновременно, обеспечивая таким образом лучшее качество жизни и минимизацию осложнений у пациентов с СД. Так, Международная федерация диабета (IDF) делает акцент на применении лекарственных препаратов, обладающих дополнительными преимуществами помимо снижения гликемии, такими как защита сердечно-сосудистой системы и почек. Примеры включают иНГЛТ2 и арГПП-1, которые снижают риски сердечно-сосудистых событий и замедляют развитие диабетической нефропатии [<xref ref-type="bibr" rid="cit10">10</xref>].</p><p>В то же время опубликованный в 2024 г. Консенсус экспертов из Великобритании, США, Китая, Индии, Австралии, ЮАР подчеркивает необходимость сохранить акцент на интенсивном снижении уровня глюкозы для достижения персонализированных целевых показателей гликемии, а не основываться исключительно на органопротективном воздействии, особенно у пациентов с коротким анамнезом СД2, не имеющих осложнений [<xref ref-type="bibr" rid="cit11">11</xref>]. Обоснованием данной позиции является прежде всего тот факт, что иНГЛТ2 и арГПП-1 продемонстрировали кардиоваскулярные преимущества у пациентов с СД2 с установленными ССЗ, ХБП и/или множественными факторами риска их развития, но не у лиц с СД2 низкого/умеренного риска. Кроме того, эффекты иНГЛТ‑2 и арГПП-1 на риски развития и прогрессии микрососудистых осложнений (нейропатии, ретинопатии) изучены недостаточно, и убедительных данных об их существенном снижении не получено [12–14].</p><p>Между тем позитивные эффекты достижения целевого контроля гликемии на риск развития и прогрессирование микрососудистых осложнений не вызывают сомнений. Адекватный контроль гликемии является универсальным фактором, обеспечивающим снижение риска развития всех компонентов серьезных неблагоприятных сердечно-сосудистых событий (Major Adverse Cardiovascular Events — МАСЕ), в то время как эффекты новых классов препаратов в отношении отдельных компонентов MACE весьма неоднородны: в то время, как арГПП-1 снижают риск инсульта и, в меньшей степени, почечных событий, иНГЛТ‑2 в основном снижают риск госпитализаций по причине сердечной недостаточности и риск неблагоприятных исходов со стороны почек [15–17]. Эта проблема частично решается комбинацией иНГЛТ2 и арГПП-1, однако экономическая эффективность комбинации иНГЛТ2 и арГПП-1 в сравнении с ранним многофакторным управлением СД2 требует подтверждения [<xref ref-type="bibr" rid="cit18">18</xref>][<xref ref-type="bibr" rid="cit19">19</xref>].</p><p>В современных условиях еще одной важной причиной ограниченного использования новых классов является их относительно высокая стоимость и не всегда хорошая переносимость. Так, в исследовании SELECT распространенность прекращения лечения составила приблизительно 30%, в то время как оценки реальной клинической практики показывают, что показатели прекращения лечения составляют от 50% до 75% через 12 месяцев. Среди основных причин отказа от продолжения терапии значатся побочные эффекты и высокая стоимость терапии [<xref ref-type="bibr" rid="cit20">20</xref>]. Эти данные поддерживают сохранение подходов к выбору терапии на основе баланса цена/эффективность/безопасность. В рекомендациях IDF и Consensus Report by the American Diabetes Association (ADA) and the European Association for the Study of Diabetes (EASD) неизменно фигурирует раздел «Лечение пациентов с СД2 типа без ССЗ и ХБП в условиях ограниченного финансирования», в котором в качестве первой линии терапии обозначается метформин, а в качестве второй — препараты сульфонилмочевины (ПСМ), — группы, с одной стороны имеющие наиболее доступную стоимость, с другой стороны — самый долговременный опыт применения [<xref ref-type="bibr" rid="cit21">21</xref>][<xref ref-type="bibr" rid="cit22">22</xref>].</p><p>Последнее особенно актуально, если учесть, что все еще нет убедительных доказательств в сохранении кардиоренальных преимуществ при долгосрочной терапии арГПП-1. Так, в мета-анализе Alexander JT и соавт., включившем 30 рандомизированных контролируемых испытаний, долговременная терапия арГПП-1 (52 недели и более) приводила к уменьшению числа инсультов (0,86; 95% ДИ 0,78–0,95, I²=0%), почечных событий (0,85; 95% ДИ 0,80–0,90, I²=0%) и смерти (RR=0,89; 95% ДИ 0,84–0,94, I²=0%) по сравнению с плацебо, однако различий между 5- и 6-компонентным MACE, фатальными или нефатальными инфарктами, сердечной недостаточностью, ретинопатией или слепотой не наблюдалось. При сравнении арГПП-1 с другими антигипергликемическими препаратами различий в частоте макроваскулярных или микроваскулярных событий выявлено не было, хотя количество пациентов, проанализированных по каждому исходу, было ограничено. Смертность также не различалась между арГПП-1 и другими антигипергликемическими препаратами (RR=0,68; 95% ДИ 0,42–1,11, I²=0%) [<xref ref-type="bibr" rid="cit23">23</xref>].</p><p>Наконец, сроки использования новых классов препаратов относительно невелики, что лишает нас возможности в текущий момент времени быть уверенными в безопасности их долгосрочного использования. Это ограничивает применение новых классов препаратов теми группами пациентов, у которых польза от их использования однозначно превышает риск.</p></sec><sec><title>Нюансы использования органопротективных сахароснижающих препаратов</title><p>Необходимо отметить, что органопротективные классы гипогликемических препаратов, при всех своих неоспоримых преимуществах, также обладают рядом существенных ограничений.</p><p>Так, эффект иНГЛТ‑2 на снижение уровня HbA1c значительно ниже, чем у других групп препаратов, таких как инсулин или арГПП-1. Например, согласно исследованию EMPA-REG OUTCOME, эмпаглифлозин снижает HbA1c примерно на 0,5% [<xref ref-type="bibr" rid="cit5">5</xref>], что недостаточно для достижения целевого уровня гликемии у многих пациентов. Отсюда вытекает следующее ограничение, подтвержденное мета-анализами: эффективность иНГЛТ‑2 в режиме монотерапии может быть недостаточной, что требует подключения дополнительных препаратов для достижения целевых показателей гликемии [<xref ref-type="bibr" rid="cit24">24</xref>]. Также необходимо упомянуть о наиболее распространенном побочном эффекте на фоне приема иНГЛТ‑2, а именно рецидивирующих урогенитальных инфекциях, обусловленных механизмом действия иНГЛТ‑2 (усилением экскреции глюкозы почками), что существенно ограничивает применение этого класса у некоторых категорий пациентов [<xref ref-type="bibr" rid="cit25">25</xref>].</p><p>АрГПП-1 представляют собой еще один органопротективный класс гипогликемических препаратов, преимущество которых заключается в улучшении контроля гликемии и снижении веса. Тем не менее, у этого класса препаратов также есть несколько ограничений. Препараты этого класса относятся к наиболее дорогим сахароснижающим препаратам, что существенно ограничивает их широкое распространение. Стоимость курса лечения варьирует в зависимости от региона, однако она остается достаточно высокой для многих пациентов [<xref ref-type="bibr" rid="cit26">26</xref>]. Кроме того, большинство препаратов группы арГПП-1 требуют ежедневных/еженедельных подкожных инъекций, что создает дискомфорт для пациентов и может снизить приверженность лечению [<xref ref-type="bibr" rid="cit27">27</xref>]. Вместе с тем с применением арГПП-1 ассоциирован ряд побочных эффектов, таких как тошнота, рвота, боль в животе, диарея, что является частой причиной досрочного прекращения терапии [<xref ref-type="bibr" rid="cit20">20</xref>]. Также необходимо отметить высокую избирательность при назначении данной группы препаратов: учитывая, что лучше всего эффект арГПП-1 раскрывается у лиц с избыточной массой тела и ожирением, применение их у лиц с нормальной массой тела может не дать ожидаемых результатов [<xref ref-type="bibr" rid="cit28">28</xref>].</p><p>Таким образом, существует большое количество клинических ситуаций, когда за невозможностью назначения органопротективных классов сахароснижающих препаратов в качестве препарата выбора в дополнение к метформину или в формате монотерапии может рассматриваться группа ПСМ [<xref ref-type="bibr" rid="cit29">29</xref>].</p></sec><sec><title>Производные сульфонилмочевины: за и против</title><p>ПСМ широко применяются в лечении СД2 уже много десятилетий и, несмотря на появление большого количества более современных классов гипогликемических препаратов, остаются важным компонентом терапевтического арсенала. Среди ключевых преимуществ ПСМ в первую очередь обозначим их мощный сахароснижающий эффект. Основные представители класса, такие как глибенкламид и глимепирид, демонстрируют способность стабильно снижать уровень HbA1c на 1–2%, что существенно превосходит многие более современные препараты [<xref ref-type="bibr" rid="cit30">30</xref>]. Бесспорным достоинством ПСМ является их низкая стоимость по сравнению с другими классами антидиабетических препаратов, что делает их доступными для широкого круга пациентов, особенно в развивающихся странах [<xref ref-type="bibr" rid="cit22">22</xref>][<xref ref-type="bibr" rid="cit31">31</xref>].</p><p>Из существенных достоинств вытекают и очевидные недостатки класса ПСМ, в первую очередь риск гипогликемии (преимущественно у пожилых лиц, получающих большое количество лекарственных препаратов, а также при совместном употреблении с алкоголем и/или нарушении режима питания) [<xref ref-type="bibr" rid="cit31">31</xref>]. Кроме того, необходимо помнить об умеренной склонности к набору веса на фоне приема ПСМ вследствие стимуляции секреции инсулина и повышения аппетита [<xref ref-type="bibr" rid="cit31">31</xref>][<xref ref-type="bibr" rid="cit32">32</xref>], а также об истощении β-клеток поджелудочной железы на фоне приема ПСМ, что приводит к необходимости усиления сахароснижающей терапии с течением времени [<xref ref-type="bibr" rid="cit33">33</xref>]. Впрочем, последнее может быть весьма успешно скорректировано добавлением к схеме лечения препарата из группы ингибиторов дипептидилпептидазы-4 (иДПП-4) [<xref ref-type="bibr" rid="cit34">34</xref>].</p><p>Безусловно, нельзя обойти вниманием и вопросы кардиологической безопасности ПСМ. В мировой литературе этот аспект обсуждается достаточно широко, однако данные исследований разных лет зачастую оказывались противоречивы из-за существенных методологических отличий, а также из-за выбора препаратов сравнения. Существуют данные, что возможные неблагоприятные эффекты ПСМ на сердечно-сосудистую систему могут быть связаны с блокировкой АТФ-зависимых калиевых каналов в кардиомиоцитах, однако подчеркивается неоднородность группы ПСМ в этом отношении. Так, по данным Chen M.H. и соавт., гликлазид и глимепирид обладают низким родством к АТФ-зависимым калиевым каналам, а их применение по сравнению с иДПП-4 не увеличивает 3P-MACE (отношение шансов (ОШ) 1,01; 95% ДИ 0,96–1,06), смертность от ССЗ (ОШ 1,01; 95% ДИ 0,93–1,08) и смертность от всех причин (ОР 0,97; 95% ДИ 0,93–1,01). При этом применение ПСМ с высоким родством к АТФ-зависимым калиевым каналам (глибенкламид, глипизид) было связано с увеличением 3P-MACE, смертности от ССЗ и от всех причин в 1,17–1,31 раза [<xref ref-type="bibr" rid="cit35">35</xref>]. Современные клинические рекомендации не случайно относят ПСМ к препаратам второй или третьей линии терапии: в вопросах кардиологической безопасности эта группа гипогликемических препаратов уступает органопротективным классам и не должна использоваться у пациентов, имеющих в анамнезе ССЗ [<xref ref-type="bibr" rid="cit29">29</xref>].</p></sec><sec><title>Ключевые преимущества и недостатки метформина</title><p>Метформин — старейший пероральный гипогликемический препарат, долгие годы представляющий собой основу всей медикаментозной терапии СД2. Способность подавлять глюконеогенез в печени и улучшать чувствительность периферических тканей к инсулину обусловливает основные достоинства метформина, а именно высокий сахароснижающий эффект (до 2,0% при монотерапии) в сочетании с низким риском развития гипогликемии и нейтральным влиянием на массу тела (или умеренным ее снижением).</p><p>К основным недостаткам метформина стоит отнести желудочно-кишечный дискомфорт, который иногда является существенным препятствием к применению препарата, а также нарушение всасывания витамина В12 в кишечнике. Наиболее серьезным (но, к счастью, редким) недостатком метформина является риск развития лактатацидоза, который ограничивает возможность применения этого препарата у некоторых уязвимых категорий пациентов, например, при тяжелой сердечной недостаточности, почечной недостаточности, алкоголизме и некоторых других состояниях.</p><p>С момента своего открытия в 1920-х гг. и до самого недавнего времени метформин предлагался в качестве препарата первой линии терапии СД2, и лишь в последние годы он постепенно оказался смещен на вторую позицию органопротективными классами препаратов [<xref ref-type="bibr" rid="cit29">29</xref>].</p></sec><sec><title>Фиксированная комбинация глибенкламида и метформина («Глюкованс®») — использование «плюсов» при уменьшении «минусов»</title><p>Одним из препаратов, удачно сочетающих в себе достоинства ПСМ и сниженный риск гипогликемии, является «Глюкованс®», который представляет собой комбинацию ПСМ 2-го поколения глибенкламида (в дозировках 2,5 мг, 5 мг) и метформина (в дозировке 500 мг). Глибенкламид в препарате «Глюкованс®» представлен в виде микронизированной формы, а уникальная система доставки частиц глибенкламида определенного размера, равномерно распределенных в растворимом матриксе метформина, способствует лучшей биодоступности и более быстрому поступлению глибенкламида в кровь после еды [<xref ref-type="bibr" rid="cit36">36</xref>].</p><p>Совместное назначение глибенкламида и метформина демонстрирует синергическое взаимодействие, приводящее к лучшему контролю гликемии, чем любым из этих препаратов в монорежиме. Исследование показало, что комбинация глибенкламида и метформина эффективнее снижает уровень HbA1c и постпрандиальной глюкозы [<xref ref-type="bibr" rid="cit37">37</xref>], причем сообщалось, что фиксированная низкодозовая комбинация глибенкламида и метформина не уступала по эффективности раздельному приему этих препаратов в более высоких дозах [<xref ref-type="bibr" rid="cit38">38</xref>]. Это делает назначение «Глюкованса®» перспективным в отношении пациентов с исходно высоким уровнем гликемии, а также тех, кто не придерживается предписанных врачом ограничений в питании.</p><p>Глибенкламид — эффективный и вместе с тем безопасный препарат из класса ПСМ. Недавние исследования показали, что глибенкламид может смягчать последствия повреждения головного мозга и повышать выживаемость после ишемического инсульта [<xref ref-type="bibr" rid="cit39">39</xref>]. Нейропротекторный потенциал глибенкламида у пациентов с инсультом объясняется не только его выраженным сахароснижающим эффектом, но и способностью глибенкламида напрямую блокировать рецепторы сульфонилмочевины 1 (Sur1) в центральной нервной системе [<xref ref-type="bibr" rid="cit40">40</xref>].</p><p>Многолетняя практика применения подтверждает безопасность и хорошую переносимость препарата. Побочные эффекты возникают реже, чем при монотерапии каждым веществом отдельно, поскольку метформин компенсирует потенциальный риск гипогликемии, связанный с приемом глибенкламида, улучшая общую толерантность к терапии [<xref ref-type="bibr" rid="cit41">41</xref>]. Одним из преимуществ «Глюкованса®» является отсутствие значительных изменений массы тела, в отличие от монотерапии глибенкламидом, который иногда ассоциируется с набором веса [<xref ref-type="bibr" rid="cit42">42</xref>]. Такое свойство делает «Глюкованс®» предпочтительным выбором для пациентов с ожирением или повышенным ИМТ, которым по разным обозначенным выше причинам невозможно назначение арГПП-1 или иНГЛТ‑2.</p><p>Наблюдения подтвердили стабильную эффективность «Глюкованса®» в течение длительного периода времени, что делает его привлекательным вариантом для долгосрочного лечения СД [<xref ref-type="bibr" rid="cit43">43</xref>].</p><p>Конечно же, нельзя не отметить удобство применения препарата «Глюкованс®». Одна таблетка представляет собой фиксированную комбинацию глибенкламида и метформина, тем самым заменяя две отдельные таблетки, что упрощает режим дозирования и повышает приверженность пациентов к терапии. Это имеет важное значение для пациентов с низким уровнем медицинской грамотности и высоким риском пропуска приема лекарственного препарата [<xref ref-type="bibr" rid="cit43">43</xref>]. Дополнительным фактором, усиливающим комплаентность пациентов, выступает экономическая доступность препарата.</p></sec><sec><title>Фиксированная комбинация глибенкламида и метформина: кому и когда?</title><p>Кому же из всего «портретного» многообразия пациентов с СД2 лучше всего предложить комбинацию глибенкламида и метформина в виде препарата «Глюкованс®»?</p><p>Согласно классификации Ahlqvist E. и соавт., предложенной в 2018 г., выделяется 5 кластеров (фенотипов) СД [<xref ref-type="bibr" rid="cit44">44</xref>]:</p><p>Разумеется, первый пункт данной классификации подразумевает под собой исключительно инсулинотерапию. Зато назначение глибенкламида в комбинации с метформином может быть многообещающим во второй подгруппе (тяжелый инсулин-дефицитный диабет). Этот кластер представлен лицами молодого и среднего возраста с выраженной гипергликемией, в том числе с моногенными формами СД, обусловленными мутациями генов KCNJ11 или ABCC8, а также более известными HNF1A или HNF4A [<xref ref-type="bibr" rid="cit45">45</xref>]. Отсутствие выраженного ожирения в этой группе исключает точку приложения для арГПП-1; применение иНГЛТ‑2 как в формате монотерапии, так и в комбинации с метформином, во-первых, может не дать желаемого сахароснижающего эффекта, а во-вторых, повышает риск развития кетоацидоза [<xref ref-type="bibr" rid="cit46">46</xref>]. Применение глибенкламида и метформина в этом случае позволит более эффективно снизить уровень гликемии благодаря мощному сахароснижающему действию ПСМ, а добавление метформина в составе фиксированной комбинации снизит риск развития гипогликемии [<xref ref-type="bibr" rid="cit45">45</xref>][<xref ref-type="bibr" rid="cit47">47</xref>]. При возникновении необходимости дополнительного усиления терапии в таких случаях можно рассмотреть назначение иНГЛТ‑2. Так, по данным G. Fulcher и соавт, добавление канаглифлозина к монотерапии ПСМ снижает уровень HbA1c на 0,74% и 0,83% при приеме в дозировках 100 мг и 300 мг соответственно в течение 18 недель [<xref ref-type="bibr" rid="cit48">48</xref>]. L. Ji и соавт сообщали, что добавление канаглифлозина в дозировке 100 мг/300 мг к комбинированной терапии метформина и ПСМ обеспечивало снижение уровня HbA1c на 0,97% и 1,06% соответственно в течение 18 недель [<xref ref-type="bibr" rid="cit49">49</xref>]. Необходимо отметить, что предлагаемое использование классификации Ahlqvist E. и соавт ни в коем случае не заменяет собой традиционную классификацию СД, а лишь дополняет ее. Дополнительная кластеризация пациентов с СД2 на основании приведенных выше критериев позволяет выделить потенциальную фенотипическую нишу, в которой перспективным может являться применение комбинации ПСМ и метформина.</p><p>Таким образом, фиксированная комбинация глибенкламида и метформина, достойным представителем которой является препарат «Глюкованс®», обладает мощным сахароснижающим эффектом при достаточном уровне безопасности и может быть использована самостоятельно или в комплексе с современными органопротективными лекарственными препаратами.</p></sec><sec><title>Заключение</title><p>Появление новых классов препаратов, таких как иНГЛТ2 и арГПП-1, расширило возможности профилактики сердечно-сосудистых и почечных осложнений, однако их высокая стоимость, ограниченная переносимость и недостаточная изученность долгосрочных эффектов диктуют необходимость рационального подхода к выбору терапии.</p><p>В этом контексте фиксированная комбинация глибенкламида и метформина («Глюкованс®») сохраняет свою актуальность как эффективный, безопасный и экономически доступный вариант лечения, особенно для пациентов с выраженной гипергликемией, ограниченным доступом к современным препаратам или низкой приверженностью к терапии. Синергическое действие компонентов «Глюкованса®» обеспечивает выраженный сахароснижающий эффект при сниженном риске гипогликемии и удобстве применения, что способствует повышению комплаенса.</p><p>Таким образом, интеграция традиционных и современных подходов позволяет достичь наилучших результатов в управлении СД2 и минимизировать риск развития осложнений.</p></sec><sec><title>Дополнительная информация</title><p>Источники финансирования.
