Поиск источников проводили в базах данных рецензируемой научной литературы РИНЦ, CyberLeninka, PubMed/MEDLINE и Google Scholar за 1990–2026 гг. по указанным ниже ключевым словам. ГП как клинический синдром включает тошноту, рвоту, чувство переполнения после еды, боль и вздутие живота при задержке опорожнения желудка (ОЖ) в отсутствие механической обструкции. Распространенность «определенного» ГП (задержка ОЖ при сцинтиграфии + симптомы ГП длительностью более 3 месяцев) в США составляет 24,2 на 100 000 человеко-лет, доля ДГП среди случаев ГП различной этиологии достигает 29,0–57,4%. При ДГП тошнота и рвота более выражены по шкале PAGI-SYM, а показатели качества жизни по SF-36 хуже, чем при ИГП. У пациентов с ГП и сахарным диабетом 1 типа по сравнению с пациентами с ГП и сахарным диабетом 2 типа реже отмечалось вздутие живота (p=0,04), было больше госпитализаций (5,1±6,4 и 2,7±5,7; p&lt;0,001), чаще выявлялась задержка ОЖ тяжелой степени (&gt;35% через 4 часа: 53,9% и 32,2%; p&lt;0,001), и симптомы ДГП проявлялись раньше (34±10 и 49±11 лет; p&lt;0,001). Общепризнан вклад автономной нейропатии с потерей энтеральных нейронов в многофакторный патогенез ДГП; также учитывают потерю интерстициальных клеток Кахаля, снижение вагальной функции и экспрессии nNOS, гипергликемию, окислительный стресс и другие факторы. Оптимальными методами диагностики задержки ОЖ при ДГП являются 4-часовая сцинтиграфия или ¹³С-дыхательный тест. Международные консенсусные рекомендации по лечению ДГП включают нутрициологическую поддержку, контроль гликемии, восстановление гидратации, применение противорвотных и прокинетических средств, а в тяжелых случаях — энтеральное или парентеральное питание и хирургическое вмешательство. Заключение: при ДГП необходим мультидисциплинарный подход, включающий раннее выявление задержки ОЖ и своевременное принятие комплексных мер, что позволит не только успешнее лечить, но и предупреждать развитие ДГП.</p></abstract><trans-abstract xml:lang="en"><sec><title>Objective</title><p>Objective: to provide an overview of the prevalence, clinical features, mechanisms and diagnosis of gastrointestinal motility disorders in diabetic gastroparesis (DGP), including a comparison with idiopathic gastroparesis (IGP), and current approaches to DGP treatment. The search was conducted in the RSCI, CyberLeninka, PubMed/MEDLINE, and Google Scholar databases for the period 1990–2026 using the keywords listed below. GP as a clinical syndrome is characterized by nausea, vomiting, postprandial fullness, pain, and bloating accompanied by delayed gastric emptying (GE) in the absence of mechanical obstruction. The prevalence of “definite” GP (delayed GE on scintigraphy + GP symptoms lasting &gt;3 months) in the United States is 24.2 per 100,000 person-years, and the proportion of DGP among cases of GP of various etiologies reaches 29.0–57.4%. In DGP, nausea and vomiting are more pronounced on the PAGI-SYM scale, and SF-36 quality-of-life scores are worse than in IGP. In patients with GP and T1DM, compared with those with GP and T2DM, bloating was less frequent (p=0.04), the number of hospitalizations was higher (5.1±6.4 vs 2.7±5.7; p&lt;0.001), severe delayed GE (&gt;35% after 4 hours) was more frequent (53.9% vs 32.2%; p&lt;0.001), and DGP symptoms appeared earlier (34±10 vs 49±11 years; p&lt;0.001). Autonomic neuropathy with loss of enteric neurons is generally recognized as contributing to the multifactorial pathogenesis of DGP; other factors include loss of interstitial cells of Cajal, reduced vagal function and nNOS expression, hyperglycemia, oxidative stress, and others. Four-hour gastric-emptying scintigraphy or a 13C breath test is optimal for diagnosing delayed GE in DGP. International consensus recommendations for DGP treatment include nutritional support, glycemic control, rehydration, antiemetics, and prokinetics; in severe cases, enteral or parenteral nutrition and surgery may be required.</p></sec><sec><title>Conclusion</title><p>Conclusion: DGP requires a multidisciplinary approach, including earlier recognition of delayed GE and prompt comprehensive measures. (FWNR-2024-0004).</funding-statement></funding-group></article-meta></front><body><sec><title>ВВЕДЕНИЕ</title><p>Численность пациентов с сахарным диабетом (СД) растет в мире и в Российской Федерации — общее количество пациентов с СД на 01.01.2025 г. составило 5 463 982 человека (3,5% населения) по данным ГНЦ Базы клинико-эпидемиологического мониторинга сахарного диабета на территории Российской Федерации [<xref ref-type="bibr" rid="cit1">1</xref>]. При СД весьма распространены симптомы со стороны верхних отделов желудочно-кишечного тракта (ЖКТ) (11–18%) [<xref ref-type="bibr" rid="cit2">2</xref>], в частности, из-за гастропареза (ГП). Цель — дать обзор распространенности, клинической картины, механизмов нарушений моторики ЖКТ и их диагностики при диабетическом ГП (ДГП), в том числе в сравнении с идиопатическим ГП, а также современных подходов к лечению ДГП. Поиск источников проводили в базах данных рецензируемой научной литературы РИНЦ, CyberLeninka, PubMed/MEDLINE и Google Scholar за 1990–2026 гг. по ключевым словам: диабетический гастропарез, клинические симптомы, патогенез, диагностика, лечение.</p></sec><sec><title>ОПРЕДЕЛЕНИЕ И ЭПИДЕМИОЛОГИЯ ГАСТРОПАРЕЗА</title><p>Замедленное опорожнение желудка, впервые описанное Boas I. в 1925 году [<xref ref-type="bibr" rid="cit3">3</xref>], Kassander P. (1958) назвал «бессимптомной задержкой желудка при сахарном диабете (gastroparesis diabeticorum)» [<xref ref-type="bibr" rid="cit4">4</xref>]. ГП — это клинический синдром, характеризующийся задержкой опорожнения желудка при отсутствии механической обструкции желудка. Основные симптомы включают тошноту, рвоту, чувство переполнения после приема пищи (раннее насыщение), боль и вздутие живота, при этом требуются объективные доказательства задержки опорожнения желудка [<xref ref-type="bibr" rid="cit5">5</xref>].</p><p>Учитывая значительное совпадение между симптомами ГП и функциональной диспепсии (ФД), которое может затруднить диагностику ГП, Консенсус Объединенной европейской гастроэнтерологии (UEG) и Европейского общества нейрогастроэнтерологии и моторики (ESNM) по ГП [<xref ref-type="bibr" rid="cit5">5</xref>], а также Согласительное совещание ESNM и российских экспертов по ГП [<xref ref-type="bibr" rid="cit6">6</xref>] определили кардинальный симптоматический паттерн ГП: тошнота и рвота являются его основными симптомами; им часто сопутствуют диспепсические симптомы наиболее распространенного подтипа ФД — постпрандиального дистресс-синдрома [<xref ref-type="bibr" rid="cit7">7</xref>].</p><p>По этиологии ГП выделяют [5–8]: ДГП, идиопатический ГП, послеоперационный ГП, лекарственный ГП, постинфекционный ГП, ГП на фоне болезни Паркинсона, коллагенозов, амилоидоза и др.; среди них частота ДГП по разным данным варьирует от 29% [<xref ref-type="bibr" rid="cit8">8</xref>][<xref ref-type="bibr" rid="cit9">9</xref>] до 57,4% [<xref ref-type="bibr" rid="cit10">10</xref>].</p><p>В историческом исследовании Jung H.K. и соавт. (2009) в округе Олмстед, штат Миннесота, в 1996–2000 гг., скорректированная по возрасту распространенность «определенного» ГП (подтвержденная при сцинтиграфии задержка опорожнения желудка + симптомы ГП более 3 месяцев) составила 24,2 на 100 000 человеко-лет (37,8 у женщин и 9,6 у мужчин), заболеваемость составила 9,8 у женщин и 2,5 у мужчин на 100 000 человеко-лет, а среди случаев «определенного» ГП причиной у 25,3% пациентов был СД [<xref ref-type="bibr" rid="cit11">11</xref>]. По результатам анализа Национальной базы данных США (2018) общая стандартизированная по полу и возрасту распространенность ГП составила 267,7 на 100 000 взрослых, распространенность ГП при СД 1 типа (СД1) составила 10,4, при СД 2 типа (СД2) — 91,0 на 100 000 человек (или 0,01% и 0,09% соответственно) [<xref ref-type="bibr" rid="cit10">10</xref>]. 10-летняя кумулятивная заболеваемость ДГП у пациентов с СД1 и СД2 составила 5,2% и 1,0% соответственно и 0,2% — у лиц без СД [<xref ref-type="bibr" rid="cit12">12</xref>].</p></sec><sec><title>КЛИНИЧЕСКАЯ ХАРАКТЕРИСТИКА ПАЦИЕНТОВ С ДГП</title><p>По данным исследований при ДГП более выражены тошнота (2,3±1,5 и 1,6±1,2, p=0,006) и рвота (1,9±1,8 и 0,9±1,4, p=0,0001) по шкале PAGI-SYM, чем при идиопатическом ГП [<xref ref-type="bibr" rid="cit13">13</xref>]. Также у пациентов с ГП и СД2 чаще отмечены симптомы вздутия живота (p=0,04) и на 70% реже — симптомы ГЭРБ, чем у пациентов с ГП и СД1 (p=0,02) [<xref ref-type="bibr" rid="cit14">14</xref>]. Пациенты с ГП и СД2 были старше (53±11 и 39±11 лет, p&lt;0,001) и имели больший ИМТ (33,4±7,5 и 26,1±6,0 кг/м², p&lt;0,001), чем пациенты с ГП и СД1 [<xref ref-type="bibr" rid="cit15">15</xref>]. У пациентов с ДГП симптомы начинались остро, при ГП и СД2 они проявлялись в более позднем возрасте (49±11 лет), чем у пациентов с ГП и СД1 (34±10 лет, p&lt;0,001) [<xref ref-type="bibr" rid="cit14">14</xref>].</p><p>ДГП в основном поражает женщин (до 70–80%), причины такого преобладания недостаточно изучены [8–16]. Пациенты с ГП и СД2 имели большее количество сопутствующих заболеваний, чем пациенты с идиопатическим ГП (5,6±3,5 и 3,7±2,8, p&lt;0,001) [<xref ref-type="bibr" rid="cit15">15</xref>]. Пациенты с ГП недиабетической природы обладают лучшей выживаемостью, чем пациенты с ДГП (p=0,0007) [<xref ref-type="bibr" rid="cit11">11</xref>].</p><p>Задержка опорожнения желудка через 4 часа значительнее при ГП с СД1 (47±27%), чем при идиопатическом ГП (28±19%) и при СД2 (33±24%; p&lt;0,001) [<xref ref-type="bibr" rid="cit15">15</xref>]. Задержка опорожнения желудка при СД в значительной степени связана с гипергликемией натощак [<xref ref-type="bibr" rid="cit16">16</xref>], с женским полом [9, 16], с повышенным уровнем HbA1c [<xref ref-type="bibr" rid="cit9">9</xref>]: риск задержки опорожнения желудка в 1,5 раза выше на каждый 1% увеличения HbA1c [<xref ref-type="bibr" rid="cit9">9</xref>]. При этом у пациентов с ГП и СД1 наблюдали на 0,9% более высокие показатели HbA1c (p=0,003) [<xref ref-type="bibr" rid="cit14">14</xref>]. Однако наличие и тяжесть симптомов ГП далеко не всегда коррелируют со скоростью опорожнения желудка [<xref ref-type="bibr" rid="cit5">5</xref>].</p><p>Шкала клинической оценки тяжести ГП по результатам его лечения (American Neurogastroenterology and Motility Society, ANMS) [<xref ref-type="bibr" rid="cit17">17</xref>] включает: 1-я степень: легкий ГП (симптомы легко контролируются, поддержка веса на обычном питании); 2-я степень: компенсированный ГП (умеренные симптомы частично контролируются, поддержка веса на лечебном питании); 3-я степень: ГП с желудочной недостаточностью (рефрактерные неконтролируемые симптомы, отсутствует поддержка веса при пероральном питании). Пациенты с СД1 чаще страдали ГП 3-й степени (49% против 39%) и реже — ГП 1-й степени (6% против 16%) по сравнению с пациентами с ГП и СД2 [<xref ref-type="bibr" rid="cit14">14</xref>]. У пациентов с ГП и СД1 отмечено больше госпитализаций за прошедший год (5,1±6,4), в основном из-за рвоты и обезвоживания, чем у пациентов с ГП и СД2 (2,7±5,7) или с идиопатическим ГП (1,6 ± 3,0, p&lt;0,001) [<xref ref-type="bibr" rid="cit15">15</xref>], больше имплантаций стимуляторов желудка [13–15] и большая частота тревожности по сравнению с пациентами с ГП и СД2 [<xref ref-type="bibr" rid="cit14">14</xref>][<xref ref-type="bibr" rid="cit15">15</xref>]. У пациентов с ГП и СД2 качество жизни в физическом домене SF-36 ниже, чем при идиопатическом ГП (34,0±10,3 и 29,6±9,2; p=0,005), в то время как у пациентов с СД1 хуже показатели домена психического здоровья, чем при идиопатическом ГП (37,6±12,5 и 34,2±12,4, p=0,03) [<xref ref-type="bibr" rid="cit15">15</xref>]. Таким образом, в целом ДГП протекает несколько тяжелее, чем идиопатический ГП, а у пациентов с ГП и СД1 клинические и моторные нарушения значительно более выражены, чем при ГП и СД2.</p></sec><sec><title>ПАТОГЕНЕЗ ДГП</title><p>Патогенез ДГП является многофакторным и включает нарушения аккомодации дна/тела желудка, релаксации пилорического отдела, антро-дуоденальной координации, а также антральную гипомоторику, гиперчувствительность к вздутию желудка и изменение деятельности тонкого кишечника; при этом задействованы несколько механизмов: потеря интерстициальных клеток Кахаля (ИКК) и энтеральных нервов, первичные фиброзные и воспалительные изменения в гладкой мускулатуре желудка, снижение функции блуждающего нерва, гипергликемия, окислительный стресс, иммунные инфильтраты, некоторые генетические факторы и др. [<xref ref-type="bibr" rid="cit5">5</xref>][18–26], хотя не все авторы подтверждают значение этих факторов в развитии ГП [<xref ref-type="bibr" rid="cit6">6</xref>].</p><p>Мезенхимальные ИКК — это пейсмейкерные клетки желудка, они генерируют спонтанные медленные электрические волны. Снижение их количества отмечено в 83% случаев у пациентов как с ДГП, так и с идиопатическим ГП [<xref ref-type="bibr" rid="cit22">22</xref>], что является результатом дисбаланса между процессами, которые поддерживают ИКК, и теми, которые повреждают их. Например, гибели ИКК способствуют потеря сигналов выживания от инсулина и инсулиноподобного фактора роста 1 (IGF-1) [<xref ref-type="bibr" rid="cit26">26</xref>], снижение экспрессии нейрональной синтазы оксида азота (nNOS), опосредующей моторику желудка путем подавления пути NRF2/фазы II [<xref ref-type="bibr" rid="cit19">19</xref>], окислительный стресс, который может быть результатом снижения уровня гемоксигеназы-1, цитопротекторного антиоксидантного фермента в макрофагах [<xref ref-type="bibr" rid="cit20">20</xref>], и др. У пациентов с ДГП потеря ИКК положительно коррелирует с задержкой твердой пищи в желудке [<xref ref-type="bibr" rid="cit23">23</xref>].</p><p>В биоптатах желудка полной толщины у пациентов с ДГП и с идиопатическим ГП выявлена утрата противовоспалительных М2-макрофагов CD206⁺ и повышение экспрессии генов, связанных с провоспалительными М1-макрофагами [<xref ref-type="bibr" rid="cit24">24</xref>], что приводит к нарушению NO-зависимой нейротрансмиссии и замедлению опорожнения желудка [<xref ref-type="bibr" rid="cit20">20</xref>].</p><p>Гипергликемия снижает фундальный тонус, стимулирует сокращение пилорического сфинктера, подавляет сокращение антрального отдела, что приводит к задержке опорожнения желудка: по результатам дыхательного теста с ¹³С-октаноевой кислотой у пациентов с СД коэффициент опорожнения желудка значительно меньше, чем в контрольной группе, причем при СД1 задержка опорожнения желудка сильнее, чем при СД2 [<xref ref-type="bibr" rid="cit25">25</xref>].</p></sec><sec><title>ДИАГНОСТИКА ДГП</title><p>В статье Jung H.K. и соавт. (2009) помимо «определенного» ГП предложено выделять «вероятный» ГП (задержка опорожнения желудка, выявленная не методом сцинтиграфии, и симптомы ГП в течение более 3 месяцев) и «возможный» ГП (либо типичные симптомы ГП без задержки опорожнения желудка, либо бессимптомная задержка опорожнения желудка) [<xref ref-type="bibr" rid="cit11">11</xref>]. Однако в современных российских и зарубежных рекомендациях для постановки диагноза ГП обязательно соблюдение трех условий: 1) наличие симптомов ГП; 2) верифицированная задержка опорожнения желудка; 3) отсутствие механических препятствий [5, 6].</p><p>До проведения диагностики ГП необходимо исключить механическую обструкцию, например, грыжи пищеводного отверстия диафрагмы, с помощью эзофагогастродуоденоскопии [<xref ref-type="bibr" rid="cit6">6</xref>], во время которой у пациентов с СД часто обнаруживают задержанную пищу в желудке, однако это не считают диагностическим признаком ГП, так как находка может быть связана с приемом препаратов, например, кислотосупрессоров [<xref ref-type="bibr" rid="cit27">27</xref>].</p><p>Сцинтиграфический тест оценки опорожнения желудка от ⁹⁹mTc-меченой твердой пищи (завтрак калорийностью 255 ккал, содержащий 2% жира [<xref ref-type="bibr" rid="cit13">13</xref>]), проводимый в течение 4 часов (задержку опорожнения желудка диагностируют, если через 4 часа в желудке остается более 10% содержимого), обеспечивает стабильность диагноза более чем в 85% случаев и является «золотым стандартом» при подозрении на нарушения опорожнения желудка [<xref ref-type="bibr" rid="cit5">5</xref>][<xref ref-type="bibr" rid="cit6">6</xref>], что подтверждено и у пациентов с ГП и СД1, и у пациентов с ГП и СД2 [<xref ref-type="bibr" rid="cit28">28</xref>]. Четырехчасовая сцинтиграфия желудка более чувствительна к опорожнению твердой пищи по сравнению с жидкостями [<xref ref-type="bibr" rid="cit5">5</xref>][<xref ref-type="bibr" rid="cit28">28</xref>]. По сравнению с идиопатическим ГП, задержка опорожнения желудка была больше у пациентов с ДГП: при сцинтиграфии пациенты с СД1 чаще имели тяжелый ГП (53,9%), чем пациенты с СД2 (32,2%) или с идиопатическим ГП (29,3%, p≤0,001) [<xref ref-type="bibr" rid="cit15">15</xref>]. При использовании дыхательного теста опорожнения желудка со стабильным (без ионизирующего излучения) изотопом ¹³С-октаноевой кислоты (gastric emptying breath test, GEBT) [<xref ref-type="bibr" rid="cit6">6</xref>] получены сходные доказательства более значительного ухудшения опорожнения желудка у пациентов с ГП и СД1 по сравнению с ГП и СД2 [<xref ref-type="bibr" rid="cit25">25</xref>].</p><p>Для оценки скорости опорожнения желудка при ДГП в высокоспециализированных центрах также применяют электрогастрографию высокого разрешения, неперевариваемую беспроводную капсулу для оценки моторики (wireless motility capsule, WMC), катетерную антропилородуоденальную манометрию высокого разрешения, магнитно-резонансную томографию, однофотонную эмиссионную компьютерную томографию, рентгенографию желудка, трансабдоминальное ультразвуковое исследование и др. [<xref ref-type="bibr" rid="cit5">5</xref>]. Однако из-за многих ограничений перечисленных методов только сцинтиграфия и ¹³С-дыхательный тест признаны достоверными тестами опорожнения желудка от твердого содержимого у пациентов с подозрением на ГП [<xref ref-type="bibr" rid="cit5">5</xref>][<xref ref-type="bibr" rid="cit6">6</xref>].</p></sec><sec><title>ЛЕЧЕНИЕ ДГП</title><p>Первым шагом в лечении симптоматического ДГП является изменение образа жизни: в рандомизированном многоцентровом исследовании интенсивное вмешательство в образ жизни у пациентов с СД позволило значительно уменьшить желудочно-кишечные симптомы в течение 4 лет наблюдения [<xref ref-type="bibr" rid="cit29">29</xref>]. Принципы лечения ДГП включают контроль веса, оптимальный мониторинг гликемии, более тщательное отслеживание приема пищи и/или манипуляции с продуктами питания, облегчение симптомов (в основном с помощью противорвотных и прокинетических средств), восстановление гидратации, а в тяжелых случаях ДГП — хирургическое лечение, чаще всего пилорические вмешательства [<xref ref-type="bibr" rid="cit5">5</xref>][<xref ref-type="bibr" rid="cit30">30</xref>].</p></sec><sec><title>Питание при ДГП</title><p>Группа российских и европейских экспертов UEG и ESNM по ГП [<xref ref-type="bibr" rid="cit5">5</xref>][<xref ref-type="bibr" rid="cit6">6</xref>][<xref ref-type="bibr" rid="cit30">30</xref>] и большинство исследователей ДГП [<xref ref-type="bibr" rid="cit8">8</xref>][<xref ref-type="bibr" rid="cit10">10</xref>][<xref ref-type="bibr" rid="cit17">17</xref>][<xref ref-type="bibr" rid="cit31">31</xref>][<xref ref-type="bibr" rid="cit32">32</xref>][<xref ref-type="bibr" rid="cit33">33</xref>][<xref ref-type="bibr" rid="cit34">34</xref>] поддержали целесообразность нутрициологического вмешательства при ДГП. Специализированные диетологические консультации проводили только у 32% пациентов с ГП и только 2% выполняли рекомендации [<xref ref-type="bibr" rid="cit31">31</xref>], тогда как в метаанализе (2024) доказано, что только одно индивидуально подобранное рациональное питание способно вдвое сократить время опорожнения желудка и в 4,4 раза повысить «клинический эффект» лечения ГП на фоне значительного улучшения гликемии и уровня HbA1c у пациентов с ДГП [<xref ref-type="bibr" rid="cit33">33</xref>].</p><p>При ДГП рекомендуется частое питание — 4–8 раз в сутки небольшими порциями; пища должна состоять из мелких частиц, ее следует тщательно пережевывать (прием пищи — в течение 30 минут). Рекомендуется принимать пищу сидя и оставаться в положении сидя в течение 1 часа после еды; ограничивать клетчатку, которая может усиливать вздутие живота и вызывать насыщение; употреблять жидкую или полужидкую пищу; жиры, обеспечивающие 20–30% калорийности суточного рациона, употреблять в жидком виде; восполнять дефицит витаминов и микроэлементов; при обильной рвоте проводить регидратацию; снижать избыточную массу тела до нормы [<xref ref-type="bibr" rid="cit5">5</xref>][<xref ref-type="bibr" rid="cit17">17</xref>][<xref ref-type="bibr" rid="cit30">30</xref>][<xref ref-type="bibr" rid="cit31">31</xref>][<xref ref-type="bibr" rid="cit32">32</xref>][<xref ref-type="bibr" rid="cit34">34</xref>]. Желательно исключить кофеин, табакокурение и алкоголь; избегать ношения тесной одежды; корректировать время приема пищи и инъекции инсулина в зависимости от уровня постпрандиальной гликемии и степени опорожнения желудка, что может значительно сократить время опорожнения желудка у пациентов с ДГП [<xref ref-type="bibr" rid="cit30">30</xref>][<xref ref-type="bibr" rid="cit32">32</xref>][<xref ref-type="bibr" rid="cit33">33</xref>].</p><p>Если все вышеперечисленные диетические вмешательства не дают результата и выявлены показания к энтеральному питанию (диабетический кетоацидоз, циклическая тошнота/рвота, потеря веса &gt;5–10% от обычной массы тела в течение 3–6 месяцев и невозможность достичь целевых показателей веса, установленных врачом, неоднократные госпитализации по поводу рефрактерного ГП, требующие внутривенной гидратации) — средством выбора является применение стандартных, иммуномодулирующих, высокобелковых или высокоэнергетических питательных смесей перорально или через еюностомическую трубку [<xref ref-type="bibr" rid="cit6">6</xref>][<xref ref-type="bibr" rid="cit17">17</xref>][<xref ref-type="bibr" rid="cit30">30</xref>], поскольку у пациентов с ДГП переваривание жидкостей в основном сохранено [<xref ref-type="bibr" rid="cit33">33</xref>]. В случае рефрактерного ДГП может потребоваться парентеральное питание [<xref ref-type="bibr" rid="cit5">5</xref>]: в проспективном исследовании показана 68%-ная выживаемость пациентов с ГП на парентеральном питании в течение 15 лет [<xref ref-type="bibr" rid="cit35">35</xref>].</p></sec><sec><title>Гликемический контроль при ДГП</title><p>Повышенная распространенность симптомов ГП при СД была в значительной степени связана с более низким уровнем гликемического контроля [<xref ref-type="bibr" rid="cit2">2</xref>][<xref ref-type="bibr" rid="cit9">9</xref>][<xref ref-type="bibr" rid="cit16">16</xref>][<xref ref-type="bibr" rid="cit17">17</xref>]. Некоторые гипогликемические препараты, например, агонисты рецепторов глюкагоноподобного пептида-1 и глюкозозависимого инсулинотропного полипептида (лираглутид, ликсисенатид, тирзепатид и др.), могут привести к задержке опорожнения желудка [<xref ref-type="bibr" rid="cit36">36</xref>], согласно официальному сайта medi.ru у пациентов с ГП в РФ применение лираглутида не рекомендуется, ликсисенатида — противопоказано, тирзепатида — противопоказано при СД1. Доказано, что эффективный гликемический контроль и коррекция времени приема пищи в зависимости от степени опорожнения желудка могут обеспечить улучшение опорожнения желудка и облегчение симптомов ГП у пациентов с СД [<xref ref-type="bibr" rid="cit5">5</xref>][<xref ref-type="bibr" rid="cit17">17</xref>][<xref ref-type="bibr" rid="cit32">32</xref>], поэтому контроль гликемии, безусловно, остается одной из основных задач у пациентов с ДГП [<xref ref-type="bibr" rid="cit30">30</xref>].</p><p>При использовании непрерывной подкожной инфузии инсулина и непрерывного мониторинга гликемии объемы переносимых жидких питательных блюд у пациентов с ДГП увеличились с 420±258 до 487±312 мл (p=0,05) за 24 недели [<xref ref-type="bibr" rid="cit37">37</xref>]. Однако в другом проспективном исследовании в течение 48 недель не удалось подтвердить связь между ухудшением симптомов со стороны ЖКТ и хроническим гликемическим контролем ни в группе пациентов с ГП и СД1, ни в группе пациентов с ГП и СД2 [<xref ref-type="bibr" rid="cit14">14</xref>]. И все же пациентам с ДГП рекомендуется строгий контроль гликемии для снижения возможного риска усугубления ГП [<xref ref-type="bibr" rid="cit17">17</xref>].</p></sec><sec><title>Фармакологическая терапия ДГП</title><p>Терапия ДГП включает несколько классов препаратов: ингибиторы протонной помпы, антагонисты дофаминовых D2-рецепторов, рецепторов 5-HT3 и NK1; агонисты рецепторов 5-HT4, 5-HT1A, мотилиновых, грелиновых рецепторов, анальгетики, трициклические антидепрессанты, СИОЗС, СИОЗСН, фитотерапия и др. [<xref ref-type="bibr" rid="cit5">5</xref>][<xref ref-type="bibr" rid="cit6">6</xref>].</p><p>Среди прокинетиков в настоящее время для лечения ДГП применяют антагонисты дофаминовых D2-рецепторов [<xref ref-type="bibr" rid="cit6">6</xref>]: метоклопрамид (единственный препарат, одобренный FDA для лечения ГП [<xref ref-type="bibr" rid="cit5">5</xref>][<xref ref-type="bibr" rid="cit17">17</xref>]) применяют по 10 мг 3–4 раза в день внутрь или при использовании назального спрея в дозе 10 мг 4 раза в день [<xref ref-type="bibr" rid="cit38">38</xref>], но не дольше 3 месяцев из-за его возможных побочных эффектов, включая удлинение интервала QT, желудочковую тахикардию, гиперпролактинемию, позднюю дискинезию и др. [<xref ref-type="bibr" rid="cit39">39</xref>]; домперидон применяют по 10–20 мг 3–4 раза в день внутрь или внутривенно; он улучшает как параметры задержки опорожнения желудка, так и симптомы диспепсии и диабетический метаболический контроль [<xref ref-type="bibr" rid="cit40">40</xref>], однако также обладает сходными вышеперечисленными побочными эффектами [<xref ref-type="bibr" rid="cit5">5</xref>][<xref ref-type="bibr" rid="cit17">17</xref>]. Метоклопрамид и домперидон зарегистрированы в РФ для облегчения симптомов тошноты и рвоты различного генеза. Пациенты с ГП и СД1 по сравнению с идиопатическим ГП чаще принимали прокинетические препараты (ОШ=2,07; p=0,03) [<xref ref-type="bibr" rid="cit15">15</xref>].</p><p>Прокинетик итоприд проявляет двойной механизм действия: антагонизм к D2-рецепторам и ингибирование ацетилхолинэстеразы [<xref ref-type="bibr" rid="cit41">41</xref>]. В нескольких проспективных [42–46], в том числе рандомизированных контролируемых исследованиях [42–44] был продемонстрирован значимый, но иногда скромный [<xref ref-type="bibr" rid="cit42">42</xref>] положительный эффект приема итоприда (в дозе 150 мг [43–46] или 200 мг/сут [<xref ref-type="bibr" rid="cit42">42</xref>]) на скорость опорожнения желудка от жидкой и твердой пищи, а также на основные симптомы ГП у пациентов с ДГП. Кроме того, в двух исследованиях было обнаружено существенное улучшение гликемических индексов (уровней HbA1c, глюкозы плазмы натощак и постпрандиальной) у пациентов с ДГП после лечения итопридом [<xref ref-type="bibr" rid="cit45">45</xref>][<xref ref-type="bibr" rid="cit47">47</xref>], хотя российские авторы не подтвердили положительной динамики показателей углеводного обмена на фоне приема итоприда [<xref ref-type="bibr" rid="cit44">44</xref>]. По официальной инструкции итоприд рекомендуется применять в РФ для лечения желудочно-кишечных симптомов, связанных с нарушением моторики желудка или его замедленным опорожнением, в частности, при ДГП [<xref ref-type="bibr" rid="cit41">41</xref>]. Однако этот препарат не включен в список рекомендаций по лечению ДГП упомянутыми выше консенсусами по гастропарезу UEG, ESNM и др. [<xref ref-type="bibr" rid="cit5">5</xref>][<xref ref-type="bibr" rid="cit6">6</xref>], поскольку симптомы ДГП не всегда успешно поддаются терапии прокинетиками [<xref ref-type="bibr" rid="cit5">5</xref>].</p><p>Поэтому в настоящее время продолжается разработка новых прокинетиков. Метопимазин, селективный, периферически ограниченный антагонист дофаминового рецептора D2, используется во Франции для симптоматического лечения тошноты и рвоты. В 2026 г. проводится II фаза двойного слепого плацебо-контролируемого рандоимизрованного контролируемого исследования (РКИ) пероральной формы мезилата метопимазина (NG101) для лечения гастропареза; препарат продемонстрировал благоприятный профиль безопасности и значительное улучшение некоторых вторичных конечных точек [<xref ref-type="bibr" rid="cit48">48</xref>]. Другой новейший препарат CIN-102 — дейтерированная форма домперидона, разрабатываемая для лечения острого рецидивирующего гастропареза, которая не оказывает клинически значимого влияния на параметры ЭКГ, включая интервал QT — проходит I фазу клинических испытаний [<xref ref-type="bibr" rid="cit49">49</xref>].</p><p>Тем пациентам с ДГП, которые плохо переносят антагонисты дофаминовых D2-рецепторов, еще в 1990 г. был предложен макролидный антибиотик и агонист мотилина эритромицин по 200–300 мг 3–4 раза в день внутрь или внутривенно; препарат усиливает сокращение антрального отдела желудка, особенно при внутривенном введении [<xref ref-type="bibr" rid="cit50">50</xref>]. Эритромицин сохранил свою актуальность до сих пор [<xref ref-type="bibr" rid="cit51">51</xref>]; лиофилизат эритромицина для приготовления раствора для внутривенного введения в РФ рекомендован для лечения ДГП. Однако продолжительность непрерывного приема препарата ограничена 1 месяцем из-за тахифилаксии [<xref ref-type="bibr" rid="cit17">17</xref>]. Другой агонист рецепторов мотилина камицинал эффективен при лечении ГП у пациентов с СД1 [<xref ref-type="bibr" rid="cit52">52</xref>]. Высокоселективные агонисты 5-HT4 — велусетраг (5–30 мг/сут) [<xref ref-type="bibr" rid="cit53">53</xref>], фелцисетраг (0,1–1,0 мг/сут) [<xref ref-type="bibr" rid="cit54">54</xref>] (три последних препарата исследуются в РФ на предмет терапии ГП), прукалоприд (2–4 мг/сут) продемонстрировали значительное улучшение показателей опорожнения желудка у пациентов с ДГП и с идиопатическим ГП [<xref ref-type="bibr" rid="cit55">55</xref>], однако другие авторы не подтвердили эффективность прукалоприда при ДГП [<xref ref-type="bibr" rid="cit56">56</xref>]. Кроме того, в соответствии с действующей в РФ инструкцией прукалоприд рекомендован лишь для лечения запоров у женщин [<xref ref-type="bibr" rid="cit6">6</xref>]. Антагонист рецептора тахикинина 1 (или нейрокинина-1) традипитант в дозе 85 мг улучшает нейротрансмиссию и уменьшает тошноту и рвоту у пациентов с ДГП [<xref ref-type="bibr" rid="cit57">57</xref>], рекомендован для лечения ГП в РФ. Трансдермальная система с антагонистом рецепторов 5-HT3 гранисетроном (3,1 мг в течение 24 часов) в течение 7 дней эффективна в уменьшении тошноты и/или рвоты у пациентов с ДГП или с идиопатическим ГП [<xref ref-type="bibr" rid="cit58">58</xref>]; препарат зарегистрирован в РФ для профилактики тошноты и рвоты при химио- или лучевой терапии.</p><p>Агонист пентапептидного рецептора грелина реламорелин (10–100 мкг подкожно 2 раза в день) [<xref ref-type="bibr" rid="cit59">59</xref>] может усиливать опорожнение желудка у пациентов с ДГП, однако он может повышать гликемию, отчасти за счет снижения секреции инсулина, стимуляции высвобождения гормона роста или улучшения усвоения углеводов [<xref ref-type="bibr" rid="cit60">60</xref>] — исследуется в РФ для лечения ДГП.</p><p>По результатам сетевого метаанализа 67 РКИ с участием 4790 пациентов с ДГП традиционная китайская фитотерапия наиболее эффективна в улучшении клинических симптомов и повышении скорости опорожнения желудка [<xref ref-type="bibr" rid="cit61">61</xref>]. Метаанализ 53 исследований с участием 4373 пациентов с ДГП показал более высокие общие показатели эффективности китайской акупунктурной терапии по сравнению с «западной» медициной (p&lt;0,00001), повышение показателей опорожнения желудка (p&lt;0,00001) и снижение уровня HbA1c (p&lt;0,00001) [<xref ref-type="bibr" rid="cit62">62</xref>].</p><p>Пероральная эндоскопическая пилоромиотомия (G-POEM) при 48-месячной оценке у пациентов с ДГП показала лучшие результаты «клинического успеха» (86,5%), чем при идиопатическом ГП (72,5%, p=0,001) [<xref ref-type="bibr" rid="cit63">63</xref>]. Несмотря на то что в РКИ интрапилорическая инъекция ботулотоксина в дозе 200 ЕД кратковременно (на 1 месяц) улучшала опорожнение желудка у пациентов с ГП, в том числе с ДГП [<xref ref-type="bibr" rid="cit64">64</xref>], в других исследованиях этот эффект не был подтвержден и применение ботулотоксина в клинической практике у пациентов с ГП не рекомендовано [<xref ref-type="bibr" rid="cit5">5</xref>]. Также при ГП получены противоречивые результаты применения электростимуляции желудка [<xref ref-type="bibr" rid="cit13">13</xref>][<xref ref-type="bibr" rid="cit14">14</xref>][<xref ref-type="bibr" rid="cit15">15</xref>][<xref ref-type="bibr" rid="cit17">17</xref>], вентиляционной гастростомии, еюностомии, транспилорического стентирования, лапароскопической пилоропластики, частичной и субтотальной гастрэктомии [<xref ref-type="bibr" rid="cit65">65</xref>]. Эксперты UEG, ESNM и Согласительного совещания по ГП из всех перечисленных подходов одобрили и признали эффективными при ГП только антагонисты дофаминовых D2-рецепторов и агонисты рецепторов 5-HT4 [<xref ref-type="bibr" rid="cit5">5</xref>][<xref ref-type="bibr" rid="cit6">6</xref>].</p><p>Неотъемлемой частью плана лечения ДГП является обучение пациента и его семьи, а также повышение осведомленности об этом состоянии. Пациентов следует проинформировать, что для поиска оптимального режима лечения может потребоваться несколько препаратов, и что более достижимой целью лечения является контроль ДГП, а не полное излечение заболевания [<xref ref-type="bibr" rid="cit29">29</xref>].</p></sec><sec><title>ЗАКЛЮЧЕНИЕ</title><p>ДГП как осложнение СД характеризуется нарушением моторики желудочно-кишечного тракта и встречается в 29–57,4% среди всех случаев ГП, преимущественно у женщин. У пациентов с ДГП наблюдались более выраженные тошнота и рвота, чем у пациентов с идиопатическим ГП. Пациенты с СД1 чаще страдали ГП тяжелой степени по сравнению с пациентами с СД2. Однако, несмотря на значительный прогресс в понимании патофизиологии ДГП и развитии объективных методов оценки желудочной дисмоторики, остаются проблемы, связанные с недостаточным пониманием этиологии ДГП и разработкой вариантов целевого лечения заболевания. Необходимы дальнейшие фундаментальные и клинические исследования ДГП для всесторонней оценки вмешательств в питание, анализа эффективности и механизмов применения различных прокинетических средств для улучшения желудочно-кишечной дисфункции, вызванной ДГП, а также разработки на этой основе персонализированного подхода к лечению и профилактике ДГП.</p></sec><sec><title>ДОПОЛНИТЕЛЬНАЯ ИНФОРМАЦИЯ</title><p>Источник финансирования.
