В последние годы происходит активное внедрение телекоммуникационных технологий в различные сферы научного и практического знания, в том числе в медицину. Сахарный диабет (СД) – одно из наиболее распространенных хронических неинфекционных заболеваний, требующих постоянного мониторинга состояния пациента. Для телемедицинского наблюдения пациентов с СД были разработаны устройства самоконтроля глюкозы с функцией передачи проведенных пациентом измерений по GSM-каналу или через сеть Интернет.</p></sec><sec><title>ЦЕЛЬ</title><p>ЦЕЛЬ. Оценка клинической эффективности телемедицинского наблюдения пациентов с СД с помощью индивидуальных устройств самоконтроля глюкозы, обладающих функцией передачи проведенных пациентом измерений по GSM-каналу или через сеть Интернет.</p></sec><sec><title>МЕТОДЫ</title><p>МЕТОДЫ. Поиск публикаций проводился в базе данных PubMed. Поиск, отбор и оценка методологического качества публикаций об оригинальных клинических исследованиях производились независимо двумя исследователями. Исследования были включены в обзор, если в них оценивалась клиническая эффективность дистанционного наблюдения пациентов с СД с помощью индивидуальных устройств самоконтроля глюкозы, обладающих функцией передачи результатов проведенных пациентом измерений по GSM-каналу или через сеть Интернет, по сравнению с традиционными методами наблюдения.</p></sec><sec><title>РЕЗУЛЬТАТЫ</title><p>РЕЗУЛЬТАТЫ. В результате систематического поиска было отобрано 10 оригинальных рандомизированных контролируемых испытаний (РКИ), обладающих различным методологическим качеством. Было выявлено, что применение дистанционных технологий в течение 12 месяцев не оказывает влияния на содержание гликированного гемоглобина, однако позволяет увеличить частоту достижения терапевтической цели у пациентов с СД 2 типа. Кроме того, было выявлено, что применение дистанционных технологий позволяет сократить частоту амбулаторных визитов пациентов с СД 2 типа, однако не приводит к сокращению длительности госпитализации данных пациентов и частоты вызова скорой медицинской помощи.</p></sec><sec><title>ЗАКЛЮЧЕНИЕ</title><p>ЗАКЛЮЧЕНИЕ. Таким образом, были получены свидетельства потенциальной терапевтической пользы дистанционного наблюдения пациентов с СД на основании данных зарубежных клинических исследований. На основании полученных результатов может быть рекомендовано проведение российского клинического исследования данной медицинской технологии.</p></sec><sec><title> </title><p> </p></sec></abstract><trans-abstract xml:lang="en"><sec><title>BACKGROUND</title><p>BACKGROUND: In recent years, there has been an active introduction of telecommunication technologies in various spheres of scientific and practical knowledge, including medicine. Diabetes mellitus (DM) is one of the most common chronic non-communicable diseases requiring constant monitoring of the patient’s condition. For telemedicine monitoring of patients with DM, glucose self-monitoring devices with the option of transmitting patient’s measurements via GSM channel or via the Internet have been developed.</p></sec><sec><title>AIM</title><p>AIM: To evaluate the clinical effectiveness of telemedicine observation of patients with DM using individual glucose self-monitoring devices with option of transmitting patient’s measurements via a GSM channel or via the Internet.</p></sec><sec><title>MATERIALS AND METHODS</title><p>MATERIALS AND METHODS: Publications about clinical trials were searched in PubMed. The search, selection and evaluation of the methodological quality of clinical trials were carried out by two independent researchers. Clinical trials were included in the review if the clinical efficacy of remote monitoring of patients with DM using individual glucose self-monitoring devices with the option of transmitting the results of measurements taken by the patient via a GSM channel or via the Internet was evaluated in comparison with traditional observation methods.</p></sec><sec><title>RESULTS</title><p>RESULTS: As a result of the systematic search, 10 original randomized controlled trials (RCTs) with different methodological quality were selected. It was found that the use of remote technologies for 12 months does not affect the level of glycated hemoglobin, however, it allows to increase the frequency of achieving individual therapeutic goals in patients with type 2 DM. In addition, it was found that the use of remote technologies allows to reduce the frequency of outpatient visits of patients with type 2 DM, but does not reduce the length of hospitalization and the frequency of an emergency calls of these patients.</p></sec><sec><title>CONCLUSION</title><p>CONCLUSION: Thus, evidence was obtained of the potential therapeutic benefits of remote observation of patients with diabetes based on data from foreign clinical studies.
