Диабетическая дистальная полинейропатия (ДДПН) является предиктором развития синдрома диабетической стопы (СДС), образования язв и ампутаций нижних конечностей, что определяет актуальность ранней верифицированной диагностики ДДПН.</p></sec><sec><title>ЦЕЛЬ</title><p>ЦЕЛЬ. Оценить возможности ранней диагностики ДДПН методом электронейромиографии.</p></sec><sec><title>МЕТОДЫ</title><p>МЕТОДЫ. Исследование проводилось на базе ФГБУ «ННИИТО им. Я.Л. Цивьяна». Всего обследовано 80 лиц – 30 человек с сахарным диабетом 2 типа (СД2), 30 – без нарушений углеводного обмена (НУО) – контрольная группа (КГ), сопоставимых по полу и возрасту, а также 20 здоровых добровольцев 20–30 лет. Лица, включенные в КГ, не имели НУО в анамнезе, уровень глюкозы в венозной плазме натощак был менее 6,0 ммоль/л, гликированного гемоглобина (НbА1с) – менее 5,7%. Все обследованные не имели симптомов и клинических признаков дистальной полинейропатии (ДПН). Участникам проведены оценка симптомов нейропатии по шкале Neuropathy Symptom Score (NSS), оценка признаков ДПН по шкале нейропатического дисфункционального счета (НДС), стимуляционная электронейромиография (ЭНМГ).</p></sec><sec><title>РЕЗУЛЬТАТЫ</title><p>РЕЗУЛЬТАТЫ. В группе СД2 по данным ЭНМГ нейропатия выявлена у 86,7% лиц. Сенсорный вариант ДДПН определен у 12 пациентов (40%), сенсомоторная нейропатия выявлена у 13 (43,3%) пациентов, у 1 пациента (3,3%) из подгруппы с СД2 выявлена моторная ДДПН. Наиболее часто поражались дополнительно обследованные медиальный и латеральный подошвенные нервы (р=0,018 и р=0,002 соответственно). В контрольной группе из 30 лиц у 9 (30%) выявлены признаки ДПН в сочетании с радикулопатией. ЭНМГ-показатели, свидетельствующие о ДДПН: латентность и амплитуда и скорости распространения возбуждения для 3 сенсорных (n. plantaris lateralis, n. plantaris medialis, n. suralis) и 2 моторных нервов (n. peroneus, n. tibialis), были значимо хуже у лиц с СД2, чем в КГ, как у обследованных с радикулопатией, так и без нее. Лица с СД2 и сенсомоторной ДДПН имели большую длительность диабета (р=0,503), выше НbА1с (р=0,077) и ИМТ (р=0,050) в сравнении с обследованными с СД2 и сенсорной нейропатией, не достигая статистической значимости.</p></sec><sec><title>ЗАКЛЮЧЕНИЕ</title><p>ЗАКЛЮЧЕНИЕ. Результаты проведенного исследования подтвердили эффективность и пригодность ЭНМГ в ранней диагностике ДДПН, а также позволили выбрать необходимые для этого оптимальные показатели ЭНМГ.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>BACKGROUND</title><p>BACKGROUND: Diabetic distal polyneuropathy (DDPN) is a predictor of diabetic foot syndrome development, ulcer formation and lower limb amputation. An early DDPN diagnosis is therefore relevant.</p></sec><sec><title>AIM</title><p>AIM: To investigate the effectiveness and suitability of electroneuromyography (ENMG) for early DDPN diagnosis.</p></sec><sec><title>MATERIALS AND METHODS</title><p>MATERIALS AND METHODS: This study was performed in the Novosibirsk Research Institute of Traumatology and Orthopaedics n.a. Ya. L. Tsivyan. A total of 80 participants were examined, including 30 DM2 patients, 30 patents without carbohydrate metabolism disorders, and 20 healthy volunteers aged 20–30 years. None of the participants showed clinical signs of distal polyneuropathy (DPN). All participants underwent evaluation of neuropathy symptoms by the Neuropathy Symptom Score scale, neurological assessment using on the scale of Neuropathic Dysfunctional Score and stimulating ENMG.</p></sec><sec><title>RESULTS</title><p>RESULTS: ENMG revealed the presence of neuropathy in 86.7% of DM2 patients. Moreover, among DM2 patients, 12 (40%) presented sensory DDPN, 13 (43.3%) presented sensorimotor neuropathy and 1 (3.3%) presented motor DDPN. The medial and lateral plantar nerves were the most commonly affected (p = 0.018 and p = 0.002, respectively). Among the 30 control individuals, 9 (30%) showed signs of DPN in combination with radiculopathy. ENMG - indicators of DDPN: latency and amplitude and speed for 3 sensory (n. Plantaris lateralis, n. Plantaris medialis, n. Suralis) and 2 motor nerves (n. Peroneus, n. Tibialis) were significantly worse in individuals with DM2 than in the control group, both for those with and without radiculopathy. Among participants with DM2, those sensorimotor DDPN tended to have a longer diabetes duration (p = 0.503), higher levels of HbA1c (p = 0.077) and higher BMI (p = 0.050) than those with sensory neuropathy, although the differences were not significant.</p></sec><sec><title>CONCLUSION</title><p>CONCLUSION: This study confirms the effectiveness and suitability of ENMG for the early diagnosis of DDPN.
