Проблема коморбидности ишемической болезни сердца (ИБС) и сахарного диабета (СД) у пациентов, нуждающихся в реваскуляризации миокарда, чрезвычайно актуальна. Впервые установленный диабет может оказать такое же неблагоприятное влияние на прогноз пациентов, подвергающихся коронарным вмешательствам, как и ранее известный СД.</p></sec><sec><title>Цель</title><p>Цель. Изучить роль впервые выявленного СД в развитии госпитальных неблагоприятных исходов коронарного шунтирования.</p></sec><sec><title>Методы</title><p>Методы. Последовательно включены 708 пациентов, подвергшихся коронарному шунтированию (КШ) в 2011–2012 году в НИИ КПССЗ. При отсутствии ранее установленного СД пациентам с пограничной гипергликемией проводился пероральный глюкозотолерантный тест (ПГТТ). Его результаты интерпретировались в соответствии с рекомендованными диагностическими критериями СД и других нарушений гликемии.</p></sec><sec><title>Результаты</title><p>Результаты. Проведение скрининга перед КШ позволило дополнительно выявить СД 2 типа (СД2) у 8,9% (n=63) обследованных пациентов, предиабет – у 10,4% (n=74). Это увеличило число пациентов с установленным СД с 15,2% (n=108) до 24,1% (n=171), число лиц с предиабетом – с 3,0% (n=21) до 13,4% (n=95), общее число лиц с любыми установленными нарушениями углеводного обмена – с 18,2% (n=129) до 37,5% (n=266). Группы не различались по числу госпитальных осложнений ввиду малого числа событий, но имел место совершенно отчетливый тренд сопоставимости впервые выявленного и установленного ранее СД в отношении частоты госпитальных осложнений КШ. При проведении регрессионного анализа известный ранее СД2 показал связь с общим числом значимых осложнений (отношение шансов (ОШ) 1,350, 95% доверительный интервал (ДИ) 1,057–1,723, р=0,020) и длительным пребыванием в стационаре (ОШ 1,609, 95% ДИ 1,202–2,155, р=0,001). Но при добавлении впервые выявленного СД в качестве вероятного предиктора описанные выше ассоциации усилились (ОШ=1,731, 95% ДИ 1,131–2,626, р=0,012 для значимых осложнений и ОШ 2,229, 95% ДИ 1,412–3,519, р&lt;0,001 для пребывания стационаре). При этом была выявлена дополнительная связь СД с риском полиорганной недостаточности (ОШ 2,911, 95% ДИ 1,072–7,901, р=0,039), неотложной операции на артериях нижних конечностей (ОШ 1,638, 95% ДИ 1,009–15,213, р=0,020) и необходимости экстракорпоральной коррекции гемостаза (ОШ 3,472, 95% ДИ 1,042–11,556, р=0,044). Следует отметить, что при учете только установленного раннее диабета связи с данными осложнениями выявлено не было.</p></sec><sec><title>Заключение</title><p>Заключение. Впервые выявленный СД так же неблагоприятен, как и установленный ранее, по степени влияния на госпитальный прогноз КШ, что подчеркивает важность активного предоперационного выявления диабета.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background: The management of coronary artery disease in patients with type 2 diabetes (T2DM) who need myocardial revascularization is a great challenge.</p></sec><sec><title>Aims</title><p>Aims: To study the role of newly diagnosed T2DM in the development of in-hospital adverse outcomes after coronary artery surgery (CABG).</p></sec><sec><title>Methods</title><p>Methods: 708 consecutive patients underwent CABG were included. All patients without history of T2DM and with border fasting hyperglycemia underwent an oral glucose tolerance test.</p></sec><sec><title>Results</title><p>Results: The screening allowed to diagnose T2DM in 8.9% and prediabetes in 10.4% of the study population. The the number of patients with T2DM increased from 15.2% to 24.1%, and with prediabetes from 3.0% to 13.4%. The total number of patients with carbohydrate metabolism disorders increased from 18.2% to 37.5%. The trend towards higher rate of in-hospital complications after CABG was defined among patients with newly diagnosed and previously diagnosed T2DM. The regression analysis demonstrated the presence of the relationships between the previously diagnosed T2DM and the total number of significant complications (odds ratio (OR) 1.350, 95% confidence interval (CI): 1.057–1.723, p=0.020) and prolonged in-hospital stay (OR 1.609, 95%CI 1.202–2.155, p=0.001). The significance of these relationships increased with the addition of newly diagnosed T2DM to the regression model (for in-hospital complications: OR 1.731, 95% CI 1.131–2.626, p=0.012; for prolonged in-hospital stay: OR 2.229, 95%CI 1.412–3.519, p&lt;0.001). Moreover, additional associations between T2DM and the risk of developing multiple organ dysfunction (OR 2.911, 95% CI 1.072–7.901, p=0.039), urgent lower extremity surgery (OR 1.638, 95%CI 1.009–15.213, p=0.020) and the need for extracorporeal correction of hemostasis (OR 3.472, 95%CI 1.042–11.556, p=0.044) have been defined. Importantly, the presence of these associations would not have been identified without including newly diagnosed DM in the regression model.</p></sec><sec><title>Conclusion</title><p>Conclusion: The newly diagnosed T2DM affects the prognosis of CABG as well as the previously diagnosed T2DM.
