относительно роли гепсидина – гормона, регулирующего метаболизм железа, в патогенезе нарушений углеводного обмена. Нарушение продукции гепсидина у пациентов с сахарным диабетом (СД) может носить двунаправленный характер и проявляться как снижением его продукции и сопровождаться синдромом перегрузки железом, так и повышением синтеза и приводить к развитию анемии хронических заболеваний. Установлено, что анемический синдром оказывает влияние на концентрацию гликированного гемоглобина, независимо от состояния углеводного обмена. Тем не менее не проводилось комплексного анализа механизмов дизрегуляции гепсидина и оценки взаимоотягощающего влияния нарушений феррокинетики и патологии углеводного обмена. Активно обсуждается вопрос ассоциации СД с развитием нейродегенеративных заболеваний, а также роль железа в патогенезе болезней Альцгеймера и Паркинсона. В данной статье приведен обзор данных зарубежной литературы касательно механизмов регуляции продукции гепсидина и нарушения обмена данного пептида при различных нозологиях, в том числе СД и нейродегенеративных заболеваниях. Представлены результаты исследований, направленных на поиск методов терапевтической коррекции нарушений феррокинетики, ассоциированных с дизрегуляцией гепсидина.</p></abstract><trans-abstract xml:lang="en"><p>Hepcidin, a hormone regulating iron metabolism, has received attention for its role in the pathogenesis of dysregulations in carbohydrate metabolism. Hepcidin disorders in patients with diabetes mellitus are bi-directional: manifesting as iron overload syndrome in cases of decreased hepcidin production and as anaemia of chronic disease in cases of hepcidin hypersecretion. However, till date, detailed analyses of mechanisms underlying hepcidin dysregulation have not been conducted nor have the interactions of ferrocinetic and carbohydrate-metabolic disorders been examined. An association between diabetes mellitus and neurodegenerative diseases as well as the role of iron metabolism in Alzheimer or Parkinson diseases is a subject of ongoing research. This review provides a summary of the current understanding of hepcidin regulation and its disorders in various diseases, including diabetes mellitus and neurodegenerative diseases.
