русла и системы микроциркуляции у пациентов с сахарным диабетом определяет специфику течения репаративного процесса, что обуславливает хронизацию раневых дефектов и в значительной степени снижает как качество жизни пациента, так и увеличивает финансовые затраты на оказание медицинской помощи данной категории больных. Успешный репаративный процесс невозможен без последовательно сменяющих друг друга фаз: воспаления, пролиферации и заживления. Кератиноциты являются основным клеточным компонентом эпидермиса, осуществляющим барьерную функцию, играют важную роль в репаративном процессе и, как показывают данные последних исследований, способны секретировать многие стероидные гормоны de novo. Повреждение целостности кожного покрова приводит к активации кератиноцитов, запускает каскад реакций, способствующих изменению фенотипа клеток эпидермиса, и приводит к их миграции и пролиферации, что происходит параллельно с изменениями способности к адгезии и конфигурации цитоскелета. Открытым остается вопрос, каким образом нарушается регуляция клеточного цикла кератиноцитов при воздействии гипергликемии и как изменяется метаболизм нервных медиаторов на различных стадиях физиологической репарации. Понимание этих процессов позволит создать необходимую научную базу для поиска и создания новых мишеней для фармакотерапии.</p></abstract><trans-abstract xml:lang="en"><p>The extent of damage to the nervous, vascular and microcirculatory systems in diabetic patients determine the regulation of physiological events that lead to the formation of chronic wounds, reduction of patient quality of life and increase of the financial value of medical care. Successful physiological repair is impossible without the successive phases of inflammation, proliferation and wound healing. Keratinocytes are the major cellular barrier components of the epidermis. These cells play an important role in physiological repair, as suggested by recent research, with many cells able to secrete steroid hormones de novo. Damage to the integrity of the skin leads to keratinocyte activation, triggering a cascade of reactions that contribute to changes in epidermal cell phenotype and lead to their proliferation and migration, analogous to changes in cellular adhesion and configuration of the cytoskeleton. An open question remains as to how the keratinocyte cell cycle, which is altered under conditions of hyperglycemia, and neurotransmitter metabolism during different stages of physiological repair are regulated.
