Связано это с тем, что были получены противоречивые данные по показателю частоты госпитализаций, связанных с СН в группе пациентов, получающих лечение препаратами из группы ингибиторов дипептидилпептидазы 4 (иДПП-4) по сравнению с плацебо. СН до недавнего времени не входила даже в перечень так называемых сосудистых или хронических осложнений СД. Хотя известно, что пациенты с СД2 в целом подвержены более высокому риску развития СН, да и риск летальности при наличии СН при диабете значительно выше, чем у лиц без нарушений углеводного обмена. Вместе с этим имеет место и обратная зависимость: среди пациентов с СН значительно чаще, чем в популяции, выявляется гипергликемия, требующая назначения антидиабетической терапии. В связи с этим представляется важным рассмотрение этой проблемы как с точки зрения патогенетических аспектов развития СН при СД2, влияния контроля гликемии и его динамики, так и способов его достижения, в том числе выбора тех или иных видов антидиабетических препаратов.</p></abstract><trans-abstract xml:lang="en"><p>Studies on the cardiovascular safety of new anti-diabetic drugs have drawn attention to the problem of heart failure (HF) in patients with type 2 diabetes mellitus (T2DM). This has included one controversial study about hospitalization associated with HF in patients treated with dipeptidyl-peptidase-4 inhibitors (I-DPP4) compared with placebo. Until recently, HF was not considered to be a vascular or chronic complication of diabetes. It is well known that patients with T2DM generally have a higher risk of developing HF. In addition, the mortality risk of patients with HF and diabetes is significantly higher than that among patients with HF without carbohydrate metabolism disorders. Among patients with HF, hyperglycaemia requiring anti-diabetic therapy is much more common than among patients without HF.
