Геномные эффекты тестостерона обусловлены длиной полиморфизма CAG-повторов гена рецептора андрогенов (AR).</p><sec><title>Цель</title><p>Цель.</p><p>Целью исследования было изучить взаимосвязь полиморфизма гена AR и показателей углеводного, липидного обменов, а также функции эндотелия у мужчин с сахарным диабетом 2 типа (СД2).</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы.</p><p>Нами было обследовано 88 мужчин в возрасте от 40 до 65 лет (средний возраст 53±6,4 лет), страдающих СД2. Всем пациентам проводили изучение углеводного и липидного обменов, исследование сосудодвигательной функции эндотелия ультразвуковой сонографией плечевой артерии, изучали биохимические маркеры эндотелиальной дисфункции ICAM-1, VCAM-1, p-селектин, e-селектин, резистин и число CAG-повторов в гене AR. Статистическую обработку данных проводили при помощи пакета прикладных статистических программ SPSS21.0 с использованием регрессионного анализа.</p></sec><sec><title>Результаты</title><p>Результаты.</p><p>Число CAG-повторов имело значимую положительную регрессию с уровнем общего тестостерона, слабую отрицательную регрессию числа CAG-повторов гена AR и показателей липидного обмена: триглицеридов, липопротеидов низкой плотности, индекса атерогенности. При оценке ультрасонографии плечевой артерии выявлены отрицательные регрессии базового диаметра плечевой артерии и линейной скорости кровотока при эндотелий-зависимой вазодилятации (ЭЗВ). Кроме того, число CAG-повторов статистически значимо коррелировало с уровнями р-селектина и резистина. Таким образом, большее количество CAG-повторов в гене рецептора AR через ослабление чувствительности к андрогенам приводит к нарушению функции эндотелия у мужчин с СД2. Увеличение числа CAG-повторов в гене рецептора AR обусловливает снижение ЭЗВ в ходе пробы с реактивной гиперемией, а также усиление продукции p-селектина и резистина.</p></sec><sec><title>Заключение</title><p>Заключение.</p><p>Таким образом, количество CAG-повторов гена рецептора AR может быть расценено как предиктор развития и прогрессии сердечно-сосудистых поражений у мужчин с СД2.</p></sec></abstract><trans-abstract xml:lang="en"><p>In recent years, actively studied the effect of androgen deficiency on the cardiovascular system, including endothelial function. Genomic effects of testosterone caused by the length of CAG repeats polymorphism in the androgen receptor (AR) gene.</p><sec><title>Aim</title><p>Aim. </p><p>To examine the association of the polymorphism in the AR gene and carbohydrate, lipid metabolism, endothelial function in men with type 2 diabetes.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. </p><p>We examined 88 men, aged 40-65 years (mean age 53±6,4years) with type 2 diabetes. All patients underwent the study of carbohydrate and lipid metabolism, the assessment of vasomotor endothelial function of the brachial artery by ultrasound sonography, were studied biochemical markers of endothelial dysfunction – ICAM-1, VCAM-1, p-selectin, e-selectin, resistin and number of CAG-repeats in the AR gene. Statistical analysis was performed using the application package SPSS 21,0 using regression analysis.</p></sec><sec><title>Results</title><p>Results. </p><p>The number of CAG repeats had a significant positive regression to the level of total testosterone, a weak negative regression of the number of CAG repeats in the AR gene and lipid metabolism: triglycerides, LDL, atherogenic index. The assessment of the brachial artery ultrasonography revealed negative regression of the baseline brachial artery diameter and blood flow velocity in the endothelium-dependent vasodilation. The number of CAG repeats was significantly correlated with the levels of p-selectin and resistin. Thus, increasing the number of CAG repeats in the AR gene via a weakening of sensitivity to androgens leads to disruption of endothelial function in men with type 2 diabetes.
