Важную роль в процессах репарации сосудистой стенки и неоваскуляризации ишемизированных тканей играютциркулирующие прогениторные клетки (ЦПК). Целью исследования было изучение влияния сопутствующего СД 2 типа (СД2) на ко-личество ЦПК у больных с ишемической болезнью сердца (ИБС) и постинфарктной сердечной недостаточностью (СН) (ишемическойкардиомиопатией). Материалы и методы.  Количество ЦПК (CD34+ клеток) определяли методом проточной цитофлюориметрии у 47 больных ИБС, из ко-торых у 14 человек ИБС сочеталась с СД2 (ИБС+СД2). 12 человек без ИБС и СД2 составили контрольную группу. Методом иммунофер-ментного анализа (ИФА) определяли концентрацию N-концевого фрагмента предшественника мозгового натрийуретического пептида(NT-proBNP), иммунореактивного инсулина (ИРИ) и С-пептида. Результаты.  Количество ЦПК в периферической крови больных с ишемической кардиомиопатией без СД2 было на 33,4% больше по срав-нению с контрольной группой. При сочетании ишемической кардиомиопатии с СД2 количество ЦПК зависело от степени компенсациидиабета. Наиболее выраженное снижение количества циркулирующих CD34+ клеток отмечалось у лиц с декомпенсированным диабетом(HbA1c=9,5?1,8%), тогда как в подгруппе больных с компенсированным/субкомпенсированным диабетом (HbA1c=6,8?0,3%) наблюда-лось значимое повышение ЦПК как по сравнению с контролем, так и по сравнению с больными с ишемической кардиомиопатией без СД2(в среднем на 46,5% (p=0,006) и 40,0% (p=0,02) соответственно). Заключение.  В периферической крови больных с ишемической кардиомиопатией, сочетающейся с СД2, количество ЦПК зависит от ком-пенсации диабета. При ИБС в сочетании с декомпенсированным диабетом (HbA1c=9,5?1,8%) наблюдается наиболее выраженное сни-жение количества циркулирующих CD34+ клеток. Количество ЦПК клеток в периферической крови больных ИБС в сочетании с СД2тем меньше, чем выше уровень глюкозы.
