Если первичные профилактические меры (компенсация углеводного обмена) не привели к предотвращению развития диабетического поражения почек, необходимо оценить, насколько быстро ожидается прогрессирование ДН, и выявить все возможные факторы, приводящие к быстрому прогрессированию этой патологии. К ним относятся, помимо традиционных факторов, гипергликемия ? НвА1с>7%, артериальная гипертония >130/80 мм рт.ст., дислипидемия), также и такой ранее мало учитываемый фактор, как анемия. Почечная анемия является следствием сниженного синтеза ЭПО интерстицием почек и развивается достаточно рано. Даже на стадии микроальбуминурии и при нормальном уровне СКФ (более 90 мл/мин) анемия может обнаруживаться у каждого пятого больного.
