Изучение уровня sICAM-1 в сыворотке крови, а также экспрессии ICAM-1 в почках у больных СД 1 с различными стадиями нефропатии. Материалы и методы. Обследовали 63 больных СД 1 (22 мужчин и 41 женщину, в возрасте 16?54 лет) при длительности заболевания от 1 мес до 34 лет. Иссле довали суточную протеинурию, альбуминурию, мочевой осадок, креатинин крови с расчетом скорости клубочковой фильтрации (СКФ) по Cockcroft-Gault, УЗИ почек, проводили cуточное мониторирование артериального давления (СМАД). Нормальная экскреция альбумина с мочой (ЭАМ) выявлена у 23 обследованных (1-я группа), у 29 зафиксирована микроальбуминурия (2-я группа), у 11 ? протеинурия (3-я группа). Типирование ICAM-1 в биоптатах почек проведено у 15 пациентов. Локализацию ICAM-1 в почечных структурах исследовали методом иммуногистохимического типирования c помощью моноклональных антител к CD54. Результаты. Уровень sICAM-1 в крови у больных СД составил 347,2?126,1 нг/мл (153?812 нг/мл), в контрольной группе ? 268,3?63,2 нг/мл (170?375 нг/мл). Средний уровень sICAM-1 у больных оказался значимо выше контроля. Больные с артериальной гипертензией имели более высокие значения sICAM-1 по сравнению с остальными пациентами. Высокий уровень sICAM-1 был характерен для больных с ранним развитием и быстрым прогрессированием нефро- и ретинопатии. Присутствие ICAM-1 в клубочках обнаружено у 7 из 15 больных СД. Заключение. Выявлена активация синтеза молекул межклеточной адгезии ICAM-1 в почках и повышение растворимой формы данных молекул (sICAM-1) в сыворотке крови у больных СД 1. Активация синтеза ICAM-1 в канальцах и клубочках почек наблюдается на самых ранних стадиях нефропатии, даже при небольшой длительности СД.
