Цель настоящего исследования состояла в оценке влияния гипергликемии на исход острого ишемического инсульта. Материалы и методы. В исследование были включены 287 больных (198 мужчин и 89 женщин) в возрасте от 46 до 78 лет (средний возраст ? 60,2?9,4 года) с ишемическим инсультом, поступивших в неврологическую клинику РГМУ в период с 2005 по 2007 г. Углеводный обмен оценивали по уровню глюкозы плазмы венозной крови, учитывая остроту ситуации и необходимость принятия неотложных мер. Сахарный диабет 2 типа был диагностирован на основании анамнеза представленной медицинской документации, включающей сведения о уровне гликемии, приеме пероральных сахароснижающих препаратов или инсулинотерапии. Уровень глюкозы в плазме венозной крови определяли натощак в первые сутки заболевания, в последующем ? на 3-е и 5-е сут инсульта. При проведении инсулинотерапии глюкозу крови исследовали несколько раз в день для достижения и поддержания стойкой нормогликемии. Резульататы. Среди обследованных пациентов у 219 больных развился каротидный инсульт (у 116 ? в системе правой и у 103 ? левой внутренней сонной артерии), у 68 больных имелся инсульт в вертебрально-базилярной системе. Была установлена связь концентрации глюкозы в сыворотке крови и выраженности неврологического дефицита как в острейшем периоде инсульта, так и в последующем. При анализе патогенетического подтипа ишемического инсульта в обеих группах оказалось, что среди больных с гликемией натощак ?7 ммоль/л значительно чаще встречались лакунарные инсульты, обусловленные поражением церебральных артерий мелкого калибра. Заключение. Гипергликемия в первые сутки инсульта ассоциирована с неблагоприятным течением заболевания в виде большей частоты летальных исходов и меньшим регрессом очагового неврологического дефицита.
