Данные для анализа получены в ходе выполнения открытой проспективной неинтервенционной на-блюдательной программы дИА-КОНТРОЛЬ?, включавшей 9844 больных СД2. В задачи исследования входили: оценка исходного состо-яния контроля гликемии по уровню HbA1c и его динамика при повторных визитах (каждые 3 месяца) в течение года, оценка структурысахароснижающей терапии и ее коррекция в зависимости от уровня HbA1c. На визите включения доля больных, имевших удовлетвори-тельный контроль гликемии (HbA1c&lt;7%), составляла 38,5%. Исходно 17% больных не получали медикаментозной сахароснижающейтерапии, 41% получали монотерапию, 19% - комбинированную терапию и 23% инсулин (моно- или в комбинации с пероральными са-хароснижающими средствами). Пациентам, впервые начинавшим лечение, наиболее часто наравне с метформином (МФ) назначалисьпрепараты из группы ингибиторов дипептидилпептидазы-4 (иДПП-4) или комбинация МФ+иДПП-4; традиционная комбинация МФ +сульфонилмочевина (СМ) чаще заменялась на более безопасную (в отношении гипогликемии) комбинацию МФ+иДДП-4; при неудовлет-ворительном контроле гликемии активно назначалась инсулинотерапия. При оценке частоты коррекции терапии в зависимости отуровня HbA1c наблюдалась естественная закономерность: чем выше уровень HbA1c, тем чаще врачи корректировали предшествующуютерапию. Однако тревожным явился тот факт, что при крайне неудовлетворительном уровне HbA1c&gt;8% более 20% врачей не менялиранее назначенную терапию, что требует более внимательного отношения к обучению врачей.</p></abstract><trans-abstract xml:lang="en"><p>This review presents a comprehensive analysis of actual ambulatory care in patients with type 2 diabetes mellitus provided in 10 federal subjects withinthe period of 1 year. The analysis is based on results from non-interventional open label prospective observational study DIA-CONTROL, whichincluded 9844 patients with DM type 2. The study was aimed to assess the initial level of glycated hemoglobin and its further dynamics according tomeasurements repeated every 3 months. Composition and efficiency of hypoglycemic therapy were also estimated in these patients.Inclusion visit showed 38.5% of patients to have adequate HbA1c levels (&lt;7%). 17% of patients were treatment, 41% remained on monotherapy,19% received combined treatment and 23% - insulin therapy (with or without oral hypoglycemic agents). Metformin and DPP-4 inhibitors,as well as their combination, were most common choices for initiation of treatment - with aforementioned combination (as more safe in respect ofhypoglycemic events) being preferred to traditional combination of metformin and sulphonylurea derivatives. Insulin therapy was actively prescribedin cases of inadequate glycemic control.Naturally, positive correlation was observed between HbA1c levels and therapy correction frequency.
