В данной статье представлены основные этапы и режимы интенсификации инсулинотерапии при сахарном диабете 2 типа (СД2). Интенсификация инсулинотерапии наиболее часто осуществляется путем присоединения к базаль- ному инсулину прандиального инсулина в основные приемы пищи с учетом постпрандиальной гликемии. Обновленный консенсус ADA/EASD (апрель 2012 г.) особую роль отводит поэтапной, пошаговой, основанной на индивидуальном целевом уровне гликемии интенсификации инсулинотерапии. Назначая инсулиновые аналоги для интенсификации инсулинотерапии пациентам с СД2, врач значительно упрощает схему обучения и самоконтроля своим пациентам.

Article reviews basic steps and regimens of insulin analogue therapy intensification in patients with type 2 diabetes mellitus (T2DM). Intensification of insulin therapy is commonly implemented by supplementation of basal insulin with prandial before breakfast, lunch and dinner. Updated EASD/ADA guidelines (August 2012) recommend individual stepwise intensification of T2DM insulin therapy. Application of insulin analogues simplifies diabetes training and patient self-monitoring.
