Центральная роль количественного определения HbA1c установлена в исследованиях основных исходов Diabetes Control Complications Trial (DCCT) и United Kingdom Prospective Diabetes Study (UKPDS). Обоснован основной метод определения риска развития осложнений в зависимости от контроля глюкозы в крови. HbA1c отражает среднюю концентрацию глюкозы крови за прошедшие 2?3 месяца. Исследования, выполняемые в разных лабораториях, являются точными и правильными в тех случаях, если их соответствующим образом контролируют и адаптируют к международным стандартам. Однако имеется ряд вопросов в отношении опубликованных результатов, в том числе проблем, связанных с самим гемоглобином (структурные и обменные нарушения) и с использованием абсолютного стандарта количественного анализа. В связи с этим рекомендуется использовать, по возможности, количественный анализ на основе высокоэффективной жидкостной хроматографии для определения фракций гемоглобина. Полагают, что случайное определение уровня глюкозы в плазме крови в клинических условиях не играет особой роли для качественного лечения больных СД на современном этапе. Если нет возможности определения уровня HbA1c, часто рекомендуют в качестве замены определять уровни глюкозы в установленное время.
