Сопоставление эффективности и переносимости 2 типов базально-болюсной терапии: растворимым аналогом инсулина длительного действия (инсулин детемир) в сочетании с быстродействующим аналогом (инсулин аспарт) и НПХ-инсулином в сочетании с человеческим инсулином короткого действия, вводимым перед приемом пищи. Материалы и методы. Открытое рандомизированное (1:1) исследование на параллельных группах больных сахарным диабетом 1 типа (595 человек) продолжительностью 18 нед. Больные получали инсулин детемир и НПХ-инсулин утром и перед сном в комбинации соответственно с инсулином аспарт и человеческим инсулином короткого действия перед едой. Результаты. Контроль гликемии при терапии инсулином детемир и инсулином аспарт улучшился по сравнению с терапией НПХ и человеческим инсулином короткого действия (HbAlc ? 7,88 против 8,11%; 95% доверительный интервал: от -0,34 до -0,10; р&lt;0,001). Гликемические профили, построенные по 8 точкам на основании результатов определения больными глюкозы в крови, также отличались (р&lt;0,001); постпрандиальный уровень глюкозы у больных, получавших инсулин детемир и инсулин аспарт, был относительно ниже. Его колебания в отдельные дни у данного больного при терапии чем у больных, получавших НПХ-инсулин и человеческий инсулин короткого действия (стандартное отклонение: 2,88 по сравнению с 3,12 ммоль/л; р&lt;0,001). Риск гипогликемии на протяжении суток и в ночное время (между 23.00 и 06.00) был соответственно на 21% (р=0,036) и 55% (р&lt;0001) меньше при терапии инсулином детемир и инсулином аспарт, чем при терапии НПХ-инсулином и человеческим инсулином короткого действия. Вес тела (в пересчете на базальный уровень HbAlc и его динамику) у больных, получавших инсулин детемир и инсулин аспарт, был на 1 кг меньше, чем при терапии НПХ-инсулином и человеческим инсулином короткого действия (р&lt;0,001). Выводы. Базально-болюсная терапия инсулином детемир в сочетании с инсулином аспарт обеспечивает более сбалансированный глике-мический контроль и лучше переносится больными, чем терапия НПХ-инсулином в сочетании с человеческим инсулином короткого действия.
