В последние годы все большее внимание уделяется возможностям использования цифровых систем для дистанционного мониторинга (ДМ) состояния здоровья пациентов.</p></sec><sec><title>ЦЕЛЬ</title><p>ЦЕЛЬ. Оценка клинической эффективности системы ДМ у пациентов с сахарным диабетом (СД) 1 и 2 типов в сравнении с традиционным амбулаторным наблюдением.</p></sec><sec><title>МАТЕРИАЛЫ И МЕТОДЫ</title><p>МАТЕРИАЛЫ И МЕТОДЫ. В период с марта по сентябрь 2024 г. проведено нерандомизированное проспективное открытое сравнительное многоцентровое исследование с параллельными группами в 7 субъектах Российской Федерации (РФ). В исследование включались пациенты с СД 1 типа (СД1), с СД 2 типа (СД2) на неинсулиновой терапии и с СД2 на терапии инсулином. Группа вмешательства использовала систему ДМ за показателями углеводного обмена, которая включала в себя глюкометр с функцией дистанционной передачи данных, мобильное приложение, в которое через технологию Bluetooth передавались данные глюкометра, а также систему передачи данных врачу. В контрольной группе оценка гликемии осуществлялась в рамках рутинной клинической практики (очные визиты пациентов с демонстрацией дневника самоконтроля).</p></sec><sec><title>РЕЗУЛЬТАТЫ</title><p>РЕЗУЛЬТАТЫ. В исследование были включены 1 572 пациента. После 180-дневного наблюдения общая доля участников, завершивших исследование, составила 48% (754 пациента). Первичная конечная точка — уровень гликированного гемоглобина (HbA1c) — снижался сопоставимо в группах ДМ у пациентов с СД1 и в обеих когортах пациентов с СД2. Доля лиц, достигших целевых значений HbA1c, была выше в группе ДМ по сравнению с контрольной группой у пациентов с СД1 (26,06% против 10,91% соответственно, р=0,023) и СД2 на неинсулиновой терапии (51,5% против 33% соответственно, р=0,003). Применение ДМ ассоциировалось со снижением внеплановых медицинских вмешательств.</p></sec><sec><title>ЗАКЛЮЧЕНИЕ</title><p>ЗАКЛЮЧЕНИЕ. Дистанционное наблюдение показало клиническую эффективность по увеличению доли пациентов, достигающих целевых значений HbA1c в группе пациентов с СД1 и СД2 на неинсулиновой сахароснижающей терапии.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>BACKGROUND</title><p>BACKGROUND: There has been an increasing focus on the use of digital systems for remote monitoring (RM) of patient health recently.</p></sec><sec><title>AIM</title><p>AIM: To evaluate the clinical effectiveness of the RM system in patients with type 1 and type 2 diabetes (T1D and T2D) compared to traditional outpatient care.</p></sec><sec><title>MATERIALS AND METHODS</title><p>MATERIALS AND METHODS: a non-randomized prospective open comparative multicenter study with parallel groups was conducted in 7 regions of the Russian Federation from March to September 2024. The study included patients with T1D, T2D on non-insulin therapy, and T2D on insulin therapy. The intervention group used a glycaemia RM system, which included a glucometer with a data transmission set, a mobile application that received data from the glucometer via Bluetooth technology, and a data transmission system for the doctor. In the control group, glycemia was assessed as part of routine clinical practice (in-person visits with a self-monitoring diary).</p></sec><sec><title>RESULTS</title><p>RESULTS: A total of 1,572 patients were included in the study. After a 180-day follow-up, the overall completion rate was 48% (754 patients). The primary endpoint, HbA1c levels, decreased comparably in the RM and control groups in patients with T1DM and in both cohorts of patients with T2DM. The proportion of individuals who achieved HbA1c target values was higher in the RM group compared to the control group in patients with T1DM (26.06% vs. 10.91%, respectively, p=0.023) and T2DM on non-insulin therapy (51.5% vs. 33%, respectively, p=0.003).
