Сахарный диабет 1 типа (СД1) — это аутоиммунное заболевание, характеризующееся нарушением функции β-клеток и дефицитом инсулина. Появляются новые данные, указывающие на нарушение регуляции оси инсулиноподобного фактора роста (ИФР) в его патофизиологии.</p></sec><sec><title>ЦЕЛЬ</title><p>ЦЕЛЬ. Изучить уровни общего ИФР-2 в сыворотке взрослых пациентов с СД1 в Ираке и корреляцию этих результатов с уровнями ИФР-1 и другими биохимическими параметрами.</p></sec><sec><title>МАТЕРИАЛЫ И МЕТОДЫ</title><p>МАТЕРИАЛЫ И МЕТОДЫ. Всего 160 участников, включая 80 пациентов с историей СД1 и 80 здоровых лиц в возрасте 18–30 лет, были набраны из Национального центра диабета, Университета Мустансирии. Все образцы крови из обеих групп были проанализированы на ИФР-1, уровень гликемии в крови натощак (ГПН), гликированный гемоглобин (HbA1c), липидный профиль и маркеры функции почек. В то же время ИФР-2 оценивался с помощью набора для иммуноферментного анализа.</p></sec><sec><title>РЕЗУЛЬТАТЫ</title><p>РЕЗУЛЬТАТЫ. Статистический анализ выявил значительные различия во всех изученных факторах между пациентами с СД1 и контрольными группами (p&lt;0,001). Примечательно, что уровни ИФР-1 были значительно ниже у пациентов с СД1 (медиана 147 мг/дл) по сравнению с контрольной группой (медиана 215 мг/дл). Для сравнения, уровни ИФР-2 были заметно повышены у пациентов (медиана 1,15 мг/дл) по сравнению с контрольной группой (медиана 0,37 мг/дл). Сильные положительные корреляции наблюдались между ГПН, HbA1c и ИФР-2, тогда как отрицательные корреляции были обнаружены между этими параметрами и ИФР-1. Кроме того, ИМТ показал положительные ассоциации с ГПН, HbA1c и ИФР-2, но отрицательные ассоциации с ИФР-1 и соотношением ИФР-1/ИФР-2.</p></sec><sec><title>ЗАКЛЮЧЕНИЕ</title><p>ЗАКЛЮЧЕНИЕ. Эти результаты подчеркивают нарушение регуляции системы ИФР при СД1 и предполагают, что ИФР-1 и ИФР-2 могут служить потенциальными биомаркерами прогрессирования заболевания и метаболической дисфункции. Необходимы дальнейшие исследования для выяснения терапевтических последствий воздействия на ось ИФР при СД1.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>BACKGROUND</title><p>BACKGROUND: Type 1 diabetes mellitus (T1D) is an autoimmune disorder characterized by impaired β-cell function and insulin deficiency, with emerging evidence implicating dysregulation of the insulin-like growth factor (IGF) axis in its pathophysiology.</p></sec><sec><title>AIM</title><p>AIM: This research aimed to explore the levels of total insulin-like growth factor-2 (IGF-2) in sera of Iraqi adult patients with T1D and correlate these findings with insulin-like growth factor-1 (IGF-1) levels and other biochemical parameters.</p></sec><sec><title>MATERIALS AND METHODS</title><p>MATERIALS AND METHODS: A total of 160 participants, including 80 patients with a history of T1D and 80 healthy controls were recruited from the National Diabetes Center, Mustansiriyah University. All Blood samples from both groups were analyzed for IGF-1, Fasting Blood Sugar (FBS), Glycated Hemoglobin (HbA1c), Lipid Profile, and Renal Function Markers. At the same time, IGF-2 was assessed via an enzyme-linked immunosorbent assay kit.</p></sec><sec><title>RESULTS</title><p>RESULTS: Statistical analysis shows significant differences in all studied factors between T1D patients and control groups (p&lt; 0.001). Notably, IGF-1 levels were significantly lower in T1D patients (median: 147 mg/dl) compared to controls (median: 215 mg/dl). In comparison, IGF-2 levels were markedly elevated in patients (median: 1.15 mg/dl) versus controls (median: 0.37 mg/dl). Strong positive correlations were observed between FBS, HbA1c, and IGF-2, whereas negative correlations were found between these parameters and IGF-1. Additionally, BMI showed positive associations with FBS, HbA1c, and IGF-2 but negative associations with IGF-1 and IGF-1/IGF-2 Ratio.</p></sec><sec><title>CONCLUSION</title><p>CONCLUSION: These findings indicate significant changes in the IGF system in patients with T1D. In particular, changes in IGF-1 and IGF-2 levels are thought to be related to the metabolic disturbances seen in T1D.
