Клинико-экономическая оценка применения препарата дапаглифлозин дополнительно к стандартной терапии у пациентов с хронической сердечной недостаточностью (ХСН) и сопутствующим сахарным диабетом 2 типа (СД2).</p></sec><sec><title>МАТЕРИАЛЫ И МЕТОДЫ</title><p>МАТЕРИАЛЫ И МЕТОДЫ. В качестве целевой популяции рассматривались все взрослые российские пациенты с подтвержденным диагнозом ХСН и сопутствующим СД2. Была проведена оценка применения дапаглифлозина дополнительно к стандартной терапии у коморбидных пациентов с ХСН и СД2 на показатели сердечно-сосудистой (СС) смертности, смертности от любых причин, госпитализации по причине ХСН, экстренного обращения по причине ХСН, а также была рассчитана величина затрат на один сохраненный год жизни и сохраненный год жизни с поправкой на качество как при применении только стандартной терапии, так и в комбинации с дапаглифлозином.</p></sec><sec><title>РЕЗУЛЬТАТЫ</title><p>РЕЗУЛЬТАТЫ. Применение дапаглифлозина в дополнение к стандартной терапии для лечения коморбидных пациентов с ХСН и СД2 в расчете на когорту в 1000 человек позволило за 1,3 года и 10 лет дополнительно предотвратить число:</p><p>При этом затраты на один год сохраненной жизни с поправкой на качество составили 1 923 509 руб. в течение первого года, 1 102 680 руб. в течение первых трех лет, и 560 841 руб. за 10 лет, что не превышало расчетное значение порога готовности платить — 3,14 млн руб., и позволило сделать вывод о клинико-экономической целесообразности применения дапаглифлозина в изучаемой популяции пациентов.</p></sec><sec><title>ЗАКЛЮЧЕНИЕ</title><p>ЗАКЛЮЧЕНИЕ. Применение дапаглифлозина в дополнение к стандартной терапии для лечения коморбидных пациентов с ХСН и СД2 клинико-экономически целесообразно, учитывая затраты на год сохраненной жизни с поправкой на качество жизни.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>OBJECTIVE</title><p>OBJECTIVE: Clinical and economic evaluation of using the dapagliflozin in addition to standard therapy for patients with CHF (Chronic Heart Failure) and concomitant type 2 DM (Diabetes mellitus).</p></sec><sec><title>MATERIALS AND METHODS</title><p>MATERIALS AND METHODS: All adult Russian patients with confirmed diagnosis of CVD and concomitant type 2 DM were considered as the target population. We evaluated the use of dapagliflozin in addition to standard therapy in comorbid patients with CHF and DM type 2 on the indicators of CC death, death from any cause of hospitalization due to CHF, emergency treatment due to CHF, as well as calculated the cost per life year saved and quality-adjusted life year (QALY) saved both when using standard therapy alone and in combination with dapagliflozin.</p></sec><sec><title>RESULTS</title><p>RESULTS: The use of dapagliflozin in addition to standard therapy for the treatment of comorbid patients with CHF and DM 2 per cohort of 1,000 people will prevent an additional 1,3 and 10 years:</p><p>At the same time, the costs per one year of quality-adjusted life saved were 1,923,509 rubles during the first year, 1,102,680 rubles during the first 3 years, and 560,841 rubles for 10 years, which did not exceed the calculated value of the willingness-to-pay threshold — 3.14 million rubles, and allowed us to conclude that dapagliflozin is clinico-economically feasible in the studied patient population.</p></sec><sec><title>CONCLUSION</title><p>CONCLUSION: The use of dapagliflozin in addition to standard therapy for the treatment of comorbid patients with CHF and type 2 diabetes is clinically and economically feasible, considering the cost per year of life saved, adjusted for quality of life.</p></sec></trans-abstract><kwd-group xml:lang="ru"><kwd>дапаглифлозин</kwd><kwd>сердечная недостаточность</kwd><kwd>сердечно-сосудистая смерть</kwd><kwd>смерть от любой причины</kwd><kwd>госпитализации по причине хронической сердечной недостаточности</kwd><kwd>экстренное обращение по причине хронической сердечной недостаточности</kwd><kwd>порог готовности платить</kwd></kwd-group><kwd-group xml:lang="en"><kwd>dapagliflozin</kwd><kwd>heart failure</kwd><kwd>cardiovascular death</kwd><kwd>death from any cause</kwd><kwd>hospitalizations due to chronic heart failure</kwd><kwd>emergency treatment due to chronic heart failure</kwd><kwd>willingness-to-pay threshold</kwd></kwd-group><funding-group><funding-statement xml:lang="ru">Работа выполнена по инициативе авторов без привлечения финансирования, публикуется при поддержке компании ООО «АстраЗенека Фармасьютикалз»</funding-statement></funding-group></article-meta></front><back><ref-list><title>References</title><ref id="cit1"><label>1</label><citation-alternatives><mixed-citation xml:lang="ru">Bertoluci MC, Rocha VZ.
