Интенсивный ежегодный прирост пациентов с СД, в основном, за счет популяции больных СД 2 типа (СД2), и диабетической нефропатией ставит перед исследователями новую цель по расширению ряда лекарственных препаратов с кардиои нефропротективным эффектами для нивелирования остаточных рисков развития и прогрессирования хронической болезни почек и ССЗ у данной когорты пациентов. Одним из таких препаратов является финеренон — новый нестероидный селективный антагонист минералокортикоидных рецепторов, гиперактивация которых опосредует процессы воспаления и фиброза в почках, ремоделирование сердца и изменение его структурных и электрических характеристик. В обзоре представлены результаты субанализа FIDELIO, посвященных механизму действия препарата, оценке эффективности финеренона, его сравнении с эффективностью терапии ингибиторами натрий-глюкозного котранспортера 2 типа и агонистами рецепторов глюкагоноподобного пептида-1 с уже доказанными органопротективными свойствами в отношении снижения риска развития событий почечной и сердечно-сосудистой конечных точек.</p></abstract><trans-abstract xml:lang="en"><p>Patients with diabetes mellitus and renal pathology are at high risk of developing end-stage kidney disease (ESKD) and cardiovascular disease (CVD), including atrial fibrillation as an life-threatening condition. The intense annual increase in patients with diabetes mellitus, mainly due to the patients with type 2 diabetes mellitus (T2D), and diabetic nephropathy sets a new goal for researchers to expand the range of drugs with cardio- and nephroprotective effects to offset the residual risks of development and progression of chronic kidney disease (CKD) and CVD in this cohort of patients. One of such drugs is finerenone — a novel selective non-steroidal mineralocorticoid receptor (MR) antagonist (MRA), hyperactivation of which mediates renal inflammation and fibrosis, cardiac remodeling and changes in its structural and electrical characteristics.
