Существуют значительные неудовлетворенные диагностические и терапевтические потребности пациентов с ХБП и СД2 в РФ, связанные с недостаточным выявлением ХБП, особенно ранних стадий, а также существенными остаточными кардиоренальными рисками на фоне терапии зарегистрированными лекарственными препаратами. Финеренон — первый селективный нестероидный антагонист минералокортикоидных рецепторов для лечения ХБП при СД2, воздействующий на отличные от других препаратов звенья патогенеза ХБП. Препарат, обладая отчетливым потенциалом снижать темпы прогрессирования дисфункции почек и ассоциированных сердечно-сосудистых событий, может существенно улучшить результаты лечения пациентов с ХБП на фоне СД2. По результатам клинических исследований кардиоренопротективный эффект финеренона был очевиден для широкого спектра стадий ХБП (от 1 до 4) у пациентов с СД2, независимо от уровня гликированного гемоглобина и терапии другими препаратами. Данный документ суммирует основные результаты анализа профилей эффективности и безопасности финеренона, проведенного в ходе Совета экспертов специалистами в области диабетологии и нефрологии, а также определяет его место в терапии пациентов с ХБП и СД2.</p></abstract><trans-abstract xml:lang="en"><p>Chronic kidney disease (CKD) in patients with type 2 diabetes (T2D) is one of the most important medical and social problems associated with significant risks for patients and a burden on the national healthcare system. There are significant unmet diagnostic and therapeutic needs of CKD in T2D patients in Russia relating to problems of underdiagnostics of CKD and substantial residual cardiorenal risks in these patients in spite of therapy with registered renoprotective medications. Finerenone is the first selective non-steroidal mineralocorticoid receptor antagonist which targets different from other drugs pathways involved in the pathogenesis of CKD in T2D. It has a significant potential to reduce residual renal and CV risks, and therefore addresses the existing therapeutic unmet need. According to the results of recent clinical studies, the cardio- and renoprotective effects of finerenone were apparent throughout a wide range of CKD C1-C4 in T2D patients, regardless of the severity of kidney dysfunction, the level of HbA1c and other drug therapy.
