Значимость сахарного диабета в качестве медико-социальной проблемы растет во всем мире. При этом большие трудности для улучшения прогноза у больных сахарным диабетом составляет высокая частота сопутствующих сердечно-сосудистых заболеваний. Для ингибитора натрий-глюкозного котранспортера 2-го типа дапаглифлозина в ходе анализа исследования CARDIA-MOS было показано снижение общей смертности на 38,6%.</p></sec><sec><title>ЦЕЛЬ</title><p>ЦЕЛЬ. Проанализировать количество добавленных лет жизни для пациентов, получающих дапаглифлозин, по сравнению с больными, не получавшими кардиопротективных сахароснижающих препаратов.</p></sec><sec><title>МАТЕРИАЛЫ И МЕТОДЫ</title><p>МАТЕРИАЛЫ И МЕТОДЫ. Для оценки частоты исходов из пациентов, включенных в регистр CARDIA-MOS, были сформированы две выборки по заранее определенным критериям: 1) пациенты, начавшие терапию дапаглифлозином в 2017 г. и получавшие его в течение 48 мес; 2) контрольная группа пациентов, не получавших кардиопротективной сахароснижающей терапии, соответствующая основной группе по ключевым показателям: возраст, длительность сахарного диабета, наличие сердечно-сосудистых заболеваний, использование инсулина, уровень гликированного гемоглобина (НЬА1с).</p></sec><sec><title>РЕЗУЛЬТАТЫ</title><p>РЕЗУЛЬТАТЫ. В популяцию пациентов, получавших дапаглифлозин, включены 499 пациентов, то же число больных отобрано в контрольную группу. Дапаглифлозин повышал ожидаемую продолжительность жизни. При этом, чем раньше назначали лечение, тем более выраженным был эффект по сравнению со стандартной терапией. Для пациентов в возрасте 60 лет дапаглифлозин добавлял 3,7 года жизни, для больных в возрасте 75 лет — 1,4 года. Показанная динамика сохранялась при анализе методом локально взвешенного сглаживания диаграммы рассеивания.</p></sec><sec><title>ЗАКЛЮЧЕНИЕ</title><p>ЗАКЛЮЧЕНИЕ. Таким образом, применение дапаглифлозина в реальной клинической практике было ассоциировано с повышением ожидаемой продолжительности жизни. Более раннее назначение дапаглифлозина приводило к большему числу добавленных лет жизни.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>BACKGROUND</title><p>BACKGROUND: Value of diabetes mellitus as social and medical issue seems to be growing worldwide. High rates of concomitant cardiovascular disease represent significant problem for prognosis improvement in diabetes mellitus patients. During analysis of CARDIA-MOS study results overall mortality decrease of 38.6% was demonstrated for sodium-glucose cotransporter-2 dapagliflozin.</p></sec><sec><title>AIM</title><p>AIM: To analyze the number of added life years in patients receiving dapagliflozin compared to patients who did not receive hypoglycemic treatment with cardioprotective effects.</p></sec><sec><title>MATERIALS AND METHODS</title><p>MATERIALS AND METHODS: To evaluate outcome rates in patients included in CARDIA-MOS register, two samples were formulated according to predetermined criteria: 1) patients who started dapagliflozin treatment in 2017 and received medication for 48 months; 2) control group of patients not receiving cardioprotective hypoglycemic treatment comparable to study group in key characteristics: age, duration of diabetes mellitus, presence of cardiovascular disease, use of insulin, glycated hemoglobin (HbA1c) levels.</p></sec><sec><title>RESULTS</title><p>RESULTS: Dapagliflozin increased life expectancy. Earlier prescription of treatment was associated with larger difference compared to standard therapy. For patients aged 60 years old dapagliflozin added 3.7 years of life, while in patients aged 75 years — 1.4 years. Demonstrated trend was consistent in analysis using locally weighted scatterplot smoothing.</p></sec><sec><title>CONCLUSION</title><p>CONCLUSION: Therefore, use of dapagliflozin in real clinical practice was associated with increase in life expectancy.
