Несмотря на прогресс в лечении пациентов с сахарным диабетом (СД), актуальной остается проблема достижения целевых значений гликемического контроля, поэтому приобретает актуальность поиск новых комплексных решений, которые могли бы усилить контроль над заболеванием и улучшить клинические исходы.</p></sec><sec><title>ЦЕЛЬ</title><p>ЦЕЛЬ. Оценить влияние разработанного комплексного подхода к управлению заболеванием на клинические и метаболические показатели пациентов с сахарным диабетом 1 и 2 типа (СД1 и СД2), участвующих в пилотной программе «НОРМА» в течение 3 и 6 мес.</p></sec><sec><title>МАТЕРИАЛЫ И МЕТОДЫ</title><p>МАТЕРИАЛЫ И МЕТОДЫ. Программа «НОРМА» — это первый российский проект комплексного ведения пациентов с СД, включающий обучение, мониторинг уровня глюкозы крови, наблюдение эндокринолога и административную поддержку. В рамках данного исследования был проведен промежуточный ретроспективный анализ данных пациентов, участвующих в программе «НОРМА», в период с октября 2020 г. по ноябрь 2021 г. В исследование были включены взрослые пациенты (≥18 лет) с СД1 или СД2 на инсулинотерапии длительностью ≥6 мес и уровнем гликированного гемоглобина (HbA1c) &gt;7,0%. Оценивались следующие показатели: социально-демографические параметры, длительность СД, наличие поздних осложнений, HbA1c, эпизоды гипогликемии, суточная доза инсулина, уровень восприятия СД (1–10 баллов) и уровень знаний (опросник из 20 вопросов).</p></sec><sec><title>РЕЗУЛЬТАТЫ</title><p>РЕЗУЛЬТАТЫ. В исследование включены 185 пациентов: 132 с СД1 и 53 с СД2, 67% из них женщины, средний возраст 41,3±14,4 года; медиана длительности СД составила 12,0 [6,0; 19,0] года; у 30 человек (16,3%) не было поздних осложнений СД. Средний уровень HbA1c снизился с 8,8±1,5% до 7,4±1,2% через 3 мес и до 7,6±1,5% через 6 мес (р&lt;0,001). Уровень HbA1c &lt;7,0% был достигнут у 38,9 и 38,1% участников через 3 и 6 мес соответственно. Суммарная суточная доза инсулина в рамках программы не изменилась. Средний уровень знаний через 3 мес Программы повысился статистически значимо — на 25,4±15,0% (р&lt;0,001). Средний уровень восприятия СД через 3 и 6 мес увеличился на 2,1±10,2% и 2,4±11,0% (р&lt;0,01) соответственно, но при этом медианы (интерквартильный размах) суммы баллов практически не изменились: 0,00% (-3,00–6,00%) и 0,50% (-3,00–9,00%) соответственно.</p></sec><sec><title>ЗАКЛЮЧЕНИЕ</title><p>ЗАКЛЮЧЕНИЕ. Внедрение интегрированного подхода к ведению пациентов с СД (включающего обучение, средства самоконтроля, медицинскую и административную поддержку) было ассоциировано с улучшением показателей гликемического контроля без значимого увеличения дозы инсулина.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>BACKGROUND</title><p>BACKGROUND: Despite progress in the treatment of patients with diabetes mellitus (DM), the problem of achieving target values of glycemic control remains relevant. In this regard, the search for new integrated solutions that could strengthen disease control and improve clinical outcomes becomes relevant.</p></sec><sec><title>AIM</title><p>AIM: To assess the impact of the developed integrated approach to disease management on the clinical and metabolic outcomes of patients with type 1 (T1DM) and type 2 diabetes mellitus (T2DM) participating in the “NORMA” pilot program for 3 and 6 months.</p></sec><sec><title>MATERIALS AND METHODS</title><p>MATERIALS AND METHODS: “NORMA” is the first Russian program designed to implement an integrated approach to DM management combines structured online education, blood glucose monitoring, supervision by endocrinologist, and administrative support. Interium retrospective analysis analysis of pre-existing data generated in the Program was performed from October 2020 to November 2021. Adults (≥18 years) with uncontrolled T1DM or T2DM on insulin (HbA1c&gt;7,0%) with duration of DM ≥6 months were included. The following characteristics were considered: social-demographic parameters, duration of DM, complications, HbA1c, hypoglycemia events, daily insulin dose, patients’ perception of DM checklist (scaled 1-10), level of DM knowledge (questionnaire of 20 points).</p></sec><sec><title>RESULTS</title><p>RESULTS: Data from 185 persons were analyzed: 132 with T1DM and 53 with T2DM, 67% women, the mean age was 41.3±14.4 years; the median DM duration was 12.0 [6.0; 19.0] years, 30 persons (16.3%) were free of any DM complications. Mean HbA1c decreased from 8.8±1.5% to 7.4±1.2% at month 3, and to 7.6±1.5% at month 6 (p&lt;0.001). HbA1c &lt;7.0% was achieved in 38.9% and 38.1% participants after 3 and 6 months, respectively. The total insulin dose has not changed within the program. The DM knowledge level after 3 months of Program increased significantly by 25.4±15.0% (p&lt;0.001).
