Диабетическая нейроостеоартропатия (ДНОАП) — инвалидизирующее осложнение сахарного диабета (СД), развивающееся вследствие нарушения периферической иннервации и характеризующееся поражением костей и суставов неинфекционного генеза.</p><p>В результате патологических переломов костей происходит формирование грубых деформаций стопы, что влечет за собой развитие хронических язвенных дефектов в зонах избыточного нагрузочного давления. Это может стать причиной локального или распространенного инфекционного воспаления в мягких тканях (флегмона) и костных структурах (остеомиелит), как следствие, ампутаций различного уровня.</p><p>В настоящее время для коррекции деформаций используются ряд хирургических вмешательств, таких как резекция костных структур, коррекция с помощью металлических конструкций и аппаратов внешней фиксации, однако данных об их отдаленных результатах в отечественной литературе практически нет. В связи с этим проведенная нами работа является актуальной.</p></sec><sec><title>ЦЕЛЬ</title><p>ЦЕЛЬ. Изучить результаты проведенных ортопедических корригирующих вмешательств у больных сахарным диабетом и нейроостеоартропатией среднего отдела стопы.</p></sec><sec><title>МАТЕРИАЛЫ И МЕТОДЫ</title><p>МАТЕРИАЛЫ И МЕТОДЫ. Проведен анализ собранных методом анкетирования данных 55 пациентов, прооперированных в отделении диабетической стопы ФГБУ «НМИЦ эндокринологии» Минздрава России с 2009-2019 гг. Всем больным была проведена реконструктивная операция, направленная на коррекцию грубой деформации среднего отдела стопы. В послеоперационном периоде проводилась иммобилизация пораженной конечности с помощью индивидуальной разгрузочной повязки Total Contact Cast (TCC) на период 3-6 мес, а в дальнейшем пациентам рекомендовались сложная ортопедическая обувь для постоянного использования и регулярный подиатрический уход.</p></sec><sec><title>РЕЗУЛЬТАТЫ</title><p>РЕЗУЛЬТАТЫ. У 16 из 55 опрошенных пациентов после первого оперативного вмешательства развился рецидив деформации, что потребовало повторной операции. Медиана времени между первой и повторной операцией составила 3 года. Язвенные дефекты оперированной стопы обнаруживались у 25 пациентов. Медиана времени между реконструктивной операцией по поводу ДНОАП и возникновением язв составила 5 лет. У 9 пациентов (17%, 95% ДИ 8-29) в последующем проводились ампутации пальцев той же стопы. Ампутации на контралатеральной конечности были выполнены у 7 пациентов (13%, 95% ДИ 5-25). Ампутации голени не проводились ни одному из пациентов, включенных в исследование.</p></sec><sec><title>ЗАКЛЮЧЕНИЕ</title><p>ЗАКЛЮЧЕНИЕ. ДНОАП — тяжелое осложнение сахарного диабета, которое может приводить к потере конечности. Для снижения рисков формирования язв и, следовательно, возможных ампутаций требуются активное наблюдение и адекватная ортопедическая помощь в послеоперационном периоде.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>BACKGROUND</title><p>BACKGROUND: Diabetic neuroosteoarthropathy is a disabling complication of diabetes mellitus that develops as a result of impaired peripheral innervation and characterized by damage to bones and joints of non-infectious origin. As a result of pathological bone fractures, gross deformities of the foot are formed, which entails development of chronic ulcerative defects in areas of excessive load pressure. Currently, a number of surgical interventions are used to correct deformities, such as resection of bone structures, correction with metal structures and external fixation devices, but there is practically no data on their long-term results in the domestic literature.</p></sec><sec><title>AIM</title><p>AIM: To study the results of orthopedic corrective interventions in patients with diabetes mellitus and midfoot neuroosteoarthropathy.</p></sec><sec><title>MATERIALS AND METHODS</title><p>MATERIALS AND METHODS: An analysis was made the data collected by questionnaire method of 55 patients operated in the diabetic foot department in the Endocrinology research centre from 2009-2019. All patients underwent a reconstructive operation aimed correcting a gross deformity of the midfoot. In postoperative period, the affected limb was off-loaded by total contact cast for a period of 3-6 months, and in the future, complex orthopedic shoes for permanent use and regular podiatric care were recommended to patients.</p></sec><sec><title>RESULTS</title><p>RESULTS: After the first surgical intervention, 16 patients developed a recurrence of deformity, which required a second surgical intervention. The median period between interventions was 3 years. Chronic wounds of various localization of the operated foot were found in 25 patients.</p><p>The median period between surgery and ulcer formation was 5 years. 9 patients with chronic wounds underwent minor amputations at the same foot. Amputations on the contralateral limb were performed in 7 patients. None of the patients underwent major amputations.</p></sec><sec><title>CONCLUSION</title><p>CONCLUSION: Charcot foot is a severe complication of diabetes mellitus that can lead to loss of a lower limb. Due to results of the research orthopedic corrections of the midfoot deformities in patients with DNOAP can reduce the risk of ulcer developement and possible major amputations.
