Известно, что так называемая «перекатная» подошва способствует снижению нагрузки на передний отдел стопы и пальцы. Подобная подошва имеется в готовой ортопедической обуви некоторых отечественных производителей, однако объективная оценка их влияния на распределение нагрузки под стопой при ходьбе не проводилась.</p></sec><sec><title>ЦЕЛЬ</title><p>ЦЕЛЬ. Изучить распределение нагрузки внутри готовой ортопедической обуви «Сурсил-Орто» в сравнении с нагрузкой внутри повседневно используемой пациентами обуви.</p></sec><sec><title>МЕТОДЫ</title><p>МЕТОДЫ. Обследованы 20 пациентов (40 стоп) с высоким риском синдрома диабетической стопы. По клиническим данным определялась «область риска» на подошвенной поверхности стопы. Внутриобувная педография (рedar, novel, Германия) выполнена в носимой пациентом обуви, затем в ортопедической обуви «Сурсил-Орто» (Москва). Оценивалось максимальное пиковое давление (МПД) для латеральной и медиальной пяточных областей, средней части стопы, латерального и медиального переднего отделов, большого пальца, II–V пальцев. Критерии эффективности: достижение МПД в зоне риска &lt;200 кПа или снижение его на 25% и более.</p></sec><sec><title>РЕЗУЛЬТАТЫ</title><p>РЕЗУЛЬТАТЫ. Выявлено значимое снижение медианы МПД в передних отделах стопы и в области риска. Доля стоп с МПД &gt;200 кПа в области риска снизилась с 58 до 30% (р=0,014), в любой области переднего отдела стопы — с 63 до 30% (p=0,04). Возрастание МПД в средних и задних отделах стопы не превышало +14% (нд). Исследуемая обувь была эффективна в 71% случаев. Значимых различий по клинико-анамнестическим параметрам между пациентами с достаточным и недостаточным эффектом не выявлено. Предикторы недостаточного эффекта: более высокое исходное МПД в области риска, преобладание больных с областью риска на большом пальце и меньшая частота области риска в латеральной части плюсны.</p></sec><sec><title>ЗАКЛЮЧЕНИЕ</title><p>ЗАКЛЮЧЕНИЕ. Испытуемая обувь с жестким предпучковым перекатом способна значимо снижать нагрузку под передним отделом и под пальцами в процессе ходьбы. В наибольшей степени разгружаются латеральная часть переднего отдела и II–V пальцы, несколько меньше — большой палец и медиальный передний отдел.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>BACKGROUND</title><p>BACKGROUND: It is known that the so-called “rocker” outsole helps to reduce the load on the forefoot and toes. Such an outsole is available in ready-made orthopedic shoes of some Russian manufacturers, however, an objective assessment of their impact on the load distribution under the foot during walking has not been carried out.</p></sec><sec><title>AIMS</title><p>AIMS: To study the pressure distribution inside the off-the-shelf orthopedic shoes“Sursil-Ortho” in comparison with the load inside the shoes used by patients on a daily basis.</p></sec><sec><title>METHODS</title><p>METHODS: We studied 20 patients (40 feet) with a high risk of diabetic foot syndrome. According to clinical data, a risk regions were determined on the plantar surface of the feet. In-shoe pedography (pedar, novel,Germany) was performed in shoes usually worn by the patients, and in orthopedic shoes“Sursil-Orto”(Moscow). The maximum peak pressure (MPP) was calculated. Criterion of efficiency: MPP in the risk region&lt;200 kPa or reducing it by 25% or more.</p></sec><sec><title>RESULTS</title><p>RESULTS: There was a significant decrease of the median MPP in the forefoot and in the risk region. The percentage of feet with MPP &gt;200 kPa in the risk region decreased from 58% to 30% (p=0,014), in any area of the forefoot — from 63% to 30% (p=0,04). The increase in MPP under the midfoot and hindfoot did not exceed +14% (ns). Tested footwear was effective in 71% of cases. Predictors of the insufficient effect were: higher initial MPP in the risk region, risk region on the hallux or in the lateral part of the forefoot.</p></sec><sec><title>CONCLUSIONS</title><p>CONCLUSIONS: Shoes with a rigid rocker outsole significantly reduce the pressure under the forefoot and under the toes during walking.
