Сахарный диабет 2 типа (СД2) и остеопороз — широко распространенные болезни среди лиц среднего и пожилого возраста. Большинство исследований остеопороза в сочетании с СД 2 типа выполнено у женщин, в то время как факторы риска снижения минеральной плотности костной ткани (МПК) у мужчин мало изучены.</p></sec><sec><title>ЦЕЛЬ</title><p>ЦЕЛЬ. Выявить факторы риска снижения МПК в позвоночнике, шейке бедра и предплечье у мужчин с СД2.</p></sec><sec><title>МЕТОДЫ</title><p>МЕТОДЫ. Обследованы 82 пациента от 50 до 75 лет, с длительностью диабета не менее одного года. Лица с известными факторами риска вторичного остеопороза в исследование не включались. Двадцать три мужчины с нормальной МПК, не имеющие СД и ожирения, составили контрольную группу. Т-критерий в поясничном отделе позвоночника, шейке бедра и предплечье недоминантной руки, а также параметры композитного состава тела оценивались с помощью двухэнергетической рентгеновской абсорбциометрии. Уровень гормонов, влияющих на костный метаболизм (паратгормон, свободный тестостерон, 25-ОН витамин D), исследован в сыворотке крови с помощью иммуноферментного анализа. Факторы риска снижения МПК оценивали с помощью многофакторного регрессионного анализа, анализа ROC-кривых.</p></sec><sec><title>РЕЗУЛЬТАТЫ</title><p>РЕЗУЛЬТАТЫ. Среди больных СД 49 человек имели нормальную МПК, у 33 выявлено снижение T-критерия как минимум в одном отделе скелета. Предиктором снижения МПК в поясничном отделе позвоночника оказался уровень свободного тестостерона &lt;5,92 пг/мл (отношение шансов (ОШ)=4,4; р=0,04). Для МПК шейки бедра факторами риска являлись масса тела &lt;95,5 кг (ОШ=2,8; р=0,04), общая масса жировой ткани (ЖТ) &lt;27 кг (ОШ=3,3; р=0,03), масса ЖТ на туловище &lt;17,5 кг (ОШ=4,5; р=0,006), масса ЖТ в центральной области живота (android fat mass) &lt;3,2 кг (ОШ=4,0; р=0,01), масса ЖТ на бедрах (gynoid fat mass) &lt;3,5 кг (ОШ=3,3; р=0,02), а также масса мягких нежировых тканей (lean mass)&lt;59 кг(ОШ=3,0; р=0,04). Факторами риска снижения МПК в предплечье были длительность СД &gt;15,5 года (ОШ=3,7; р=0,03) и уровень гликированного гемоглобина (HbA1c) &lt;8,15% (ОШ=3,8; р=0,03). Паратгормон и 25-ОН витамин D не являлись независимыми предикторами МПК.</p></sec><sec><title>ЗАКЛЮЧЕНИЕ</title><p>ЗАКЛЮЧЕНИЕ. У мужчин с СД 2 типа низкий уровень свободного тестостерона является фактором риска снижения МПК в поясничном отделе позвоночника, а длительность СД — фактором риска снижения МПК в предплечье. Наличие ожирения ассоциировано с повышением МПК в шейке бедра; высокий уровень HbА1с связан с повышением МПК в предплечье.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>BACKGROUND</title><p>BACKGROUND: Type 2 diabetes and osteoporosis are widespread diseases in the middle-aged and elderly people. Most studies of osteoporosis in patients with type 2 diabetes have been performed in women; meantime risk factors for lowering bone mineral density (BMD) in men have been little studied.</p></sec><sec><title>AIMS</title><p>AIMS: to identify risk factors for decreased BMD at the lumbar spine, femoral neck and forearm in men with type 2 diabetes.</p></sec><sec><title>METHODS</title><p>METHODS: Eighty two men from 50 to 75 years old, with duration of diabetes for at least one year, were included in the study. Individuals with known risk factors for secondary osteoporosis were not included. Twenty-three men with normal BMD having no diabetes or obesity were acted as control. The T-score at the lumbar spine, femoral neck and forearm of a non-dominant arm, as well as body composition parameters, were evaluated by dual-energy X-ray absorptiometry. The levels of hormones that affect bone metabolism (parathyroid hormone, free testosterone, 25-OH vitamin D) were measured in blood serum by ELISA. Risk factors for reducing BMD were identified using multivariate regression analysis and receiver operating characteristic (ROC) curves.</p></sec><sec><title>RESULTS</title><p>RESULTS: Among patients with diabetes, 49 individuals had normal BMD and 33 showed decreased T-score values (&lt;-1 SD). Free testosterone &lt;5.92 pg/ml was predictor for decreased BMD at the lumbar spine (OR=4.4, p=0.04). For femoral neck, the risk factors were body weight &lt;95.5 kg (OR=2.8, p=0.04), total fat mass &lt;27 kg (OR=3.3, p=0.03), truncal fat mass&lt;17.5 kg(OR=4.5, p=0.006), android (central abdominal) fat mass &lt;3.2 kg(OR=4.0, p=0.01), gynoid (hip) fat mass &lt;3.5 kg(OR=3.3, p=0.02), and lean mass &lt;59 kg(OR=3.0, p=0.04). Risk factors for reduced BMD at the forearm were diabetes duration&gt;15.5 years (OR=3.7, p=0.03) and HbA1c &lt;8.15% (OR=3.8, p=0.03). Parathyroid hormone and 25-OH-vitamin D did not predict BMD independently.</p></sec><sec><title>CONCLUSIONS</title><p>CONCLUSIONS: In men with type 2 diabetes, low free testosterone is a risk factor for decreased BMD in the lumbar spine, and diabetes duration is a risk factor for decreased BMD in the forearm.
