Золотым стандартом хирургического лечения пациентов c далекозашедшей стадией пролиферативной диабетической ретинопатии (ПДР) является витреоретинальная хирургия (ВРХ). При этом вопрос о сроках удаления начальной катаракты у данной категории больных остается открытым до настоящего времени.</p></sec><sec><title>ЦЕЛЬ</title><p>ЦЕЛЬ. Изучить эффективность выполнения факоэмульсификации катаракты (ФЭК) вторым этапом при удалении силиконового масла (СМ) после ВРХ у пациентов с далекозашедшей стадией ПДР.</p></sec><sec><title>МЕТОДЫ</title><p>МЕТОДЫ. В исследование включены 67 пациентов с ПДР и осложненной начальной катарактой, разделенных на две группы. I группа: первый этап — ВРХ с силиконовой тампонадой, второй этап — ФЭК c имплантацией интраокулярной линзы (ИОЛ) одновременно с плановым удалением СМ. II группа: первый этап — ФЭК с имплантацией ИОЛ одномоментно с ВРХ+силиконовая тампонада, второй этап — удаление СМ из витреальной полости.</p></sec><sec><title>РЕЗУЛЬТАТЫ</title><p>РЕЗУЛЬТАТЫ. Улучшение зрительных функций в I группе достигнуто у 88,6%, во II — у 68,7% пациентов.</p></sec><sec><title>ЗАКЛЮЧЕНИЕ</title><p>ЗАКЛЮЧЕНИЕ. Результаты предварительных исследований свидетельствуют об эффективности выполнения ФЭК вторым этапом после ВРХ во время удаления СМ у пациентов с далекозашедшей стадией ПДР и осложненной начальной катарактой. Это обеспечивает более щадящее отношение к анатомическим структурам глаза во время первого этапа (ВРХ), способствуя уменьшению числа интраи послеоперационных осложнений.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>BACKGROUND</title><p>BACKGROUND: The «gold standart» for surgical treatment of patients with proliferative diabetic retinopathy (PDR is vitreoretinal surgery. However, the question of the timing of the removal of primary cataract in this category of patients remains open.</p></sec><sec><title>AIM</title><p>AIM: To evaluate the efficacy of phacoemulsification of primary cataract by the second stage after vitreoretinal surgery of PDR patients</p></sec><sec><title>METHODS</title><p>METHODS: 67 cases of surgery treatment of patients with PDR and complicated primary cataract were enrolled. This patients were divided into two groups depending on the treatment tactics. In the 1st group patients were subjected to a two–step surgical procedure: vitreoretinal surgery with silicone oil tamponade performed as the 1st step in their treatment; followed by the 2d step, phacoemulsifi tion surgery and silicone oil removal, and the IOL implantation, respectively. In the 2d group phacoemulsifi tion performed simultaneously with vitreoretinal surgery: phacoemulsification, IOL implantation, vitreoretinal surgery with silicone oil tamponade. The second step differed in the removal of silicone oil from the vitreous cavity.</p></sec><sec><title>RESULTS</title><p>RESULTS: Visual functions improved in 88.6% of cases in group 1, and in 68.7% in group 2.</p></sec><sec><title>CONCLUSIONS</title><p>CONCLUSIONS: Outcomes of the preliminary studies suggest that it is more viable to perform phacoemulsification surgery sometime later along with silicone oil removal on PDR patients with complicated primary cataract.
