Особенностью лечения пациенток с любой формой сахарного диабета (СД) во время беременности является необходимость достижения максимально близких к физиологическим значений уровня гликемии как основного условия благополучного течения и исходов беременности. Для выполнения этой задачи необходим поиск эффективных и безопасных режимов инсулинотерапии.</p></sec><sec><title>ЦЕЛЬ</title><p>ЦЕЛЬ. Изучить состояние гликемического профиля и исходы беременности у женщин с СД 1 типа (СД1), использовавшим режим постоянной подкожной инфузии инсулина (ППИИ) и режим множественных инъекций инсулина (МИИ).</p></sec><sec><title>МЕТОДЫ</title><p>МЕТОДЫ. Проведено непрерывное мониторирование глюкозы (НМГ) 100 беременным с СД1, использовавшим ППИИ, и 100 женщинам, использовавшим МИИ для оценки эффективности данных режимов инсулинотерапии. Проведены анализ времени нахождения пациенток в целевом диапазоне уровня глюкозы и определение индексов вариабельности гликемии MAGE, MODD и CONGA в I, II и III триместрах беременности.</p></sec><sec><title>РЕЗУЛЬТАТЫ</title><p>РЕЗУЛЬТАТЫ. При использовании режима ППИИ достигнута значимо более низкая концентрация HbA1c в I, II и III триместрах беременности – 6,3 [5,9; 7,2]%, 5,9 [5,6; 6,1]% и 5,8 [5,6; 6,2]% по сравнению с группой МИИ – 7,0 [6,2; 7,5]%, 6,3 [5,5; 6,8]%, 6,3 [5,9; 6,7]%. Улучшились показатели вариабельности гликемии (индексов MAGE, MODD и CONGA) уже со II триместра беременности у пациенток с СД1, использовавших режим ППИИ, что не было отмечено у женщин, использовавших режим МИИ. Длительность гипергликемии по результатам НМГ в группе беременных с СД1, использовавших ППИИ, составила 25 [13; 38]% и была ниже этого показателя по сравнению с группой женщин, использовавших МИИ, – 41 [18; 54]% (р&lt;0,01). Отсутствовал риск акушерских и перинатальных осложнений при длительности гипергликемического состояния менее 25% времени проведения НМГ, тогда как риск неонатальной гипогликемии появлялся при длительности гипогликемического состояния у матери с СД1 ≥0,2% времени НМГ. Выявлена связь вариабельности гликемии (индексы MAGE и MODD) с риском развития макросомии, подтверждена зависимость вариабельности гликемии (индексы MODD и CONGA) риск развития неонатальной гипогликемии и преэклампсии.</p></sec><sec><title>ЗАКЛЮЧЕНИЕ</title><p>ЗАКЛЮЧЕНИЕ. Комплексная оценка гликемического профиля при использовании НМГ подтвердила преимущества использования ППИИ у беременных с СД1 в достижении целевых значений гликемии, снижении вариабельности гликемии, длительности гипогликемических эпизодов, что привело к снижению частоты акушерских и перинатальных осложнений.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>BACKGROUND</title><p>BACKGROUND: Patients with any form of diabetes during pregnancy should achieve the target (close to physiological) values of glycaemia, the main condition for a safe course and outcomes of pregnancy. To accomplish this task, effective and safe methods of insulin therapy should be selected.</p></sec><sec><title>AIM</title><p>AIM: To determine the glycaemic profile and pregnancy outcomes in women with type 1 diabetes treated with continuous subcutaneous insulin infusion (CSII) and multiple insulin injections (MII).</p></sec><sec><title>METHODS</title><p>METHODS: A continuous glucose monitoring (CGM) of 100 pregnant women with type 1 diabetes treated with CSII and 100 women treated with MII was conducted to assess the effectiveness of these insulin therapy regimens in achieving target blood glucose values.</p></sec><sec><title>RESULTS</title><p>RESULTS: HbA1c levels were significantly lower during the first, second, and third trimesters in patients treated with CSII than those treated with MII. Glucose variability has already improved since the second trimester of pregnancy in women treated with CSII, which was not observed in those treated with MII. The period of hyperglycaemia according to the results in pregnant women treated with CSII was 25 [13; 38] %, which was lower than those treated with MII, 41 [18; 54] %. No risk of obstetric and perinatal complications was observed with the duration of the hyperglycaemic state of &lt;25% of the CGM time, whereas the risk of neonatal hypoglycaemia appeared with the duration of the hypoglycaemic state of a mother with type 1 diabetes of &gt;0.2%.
